胃肠道肿瘤病人术后早期应用不同肠内营养制剂的比较研究 被引量:9

摘要 目的:比较研究两种不同肠内营养(EN)制剂在胃肠道肿瘤手术病人术后早期EN支持中的应用效果。方法:将68例胃肠道手术病人随机分为康全甘组(n=34)和能全素组(n=34),术后48 h起即给予EN支持,分别于术前和术后第7天测定血清清蛋白(ALB)、前清蛋白(PA)和转铁蛋白(TF)水平,并计算病人体质指数(BMI),同时观察记录病人术后肛门排气时间和并发症例数,测定病人的负氮平衡。结果:康全甘组病人血清PA水平、负氮平衡等好于能全素组,而腹泻等并发症少于能全素组,其余指标无显著性差异。结论:康全甘是一种脂肪吸收不良专用型的EN制剂,能提高肠道对EN的耐受性,更好地改善病人术后的营养状况。 Objective : To compare the effectiveness of two kinds of early postoperative enteral nutrition in the Use of patients after gastrointestinal surgery. Methods : Totally 68 postoperative patients of gastrointestinal carcinoma from January 2010 to January 2011 in our hospital were randomly divided into Nutrison group ( n = 34) and Nutrison MCT group ( n = 34 ). Each group received nutritional support of the same calorie from 48 hours postoperation. On the day before operation and the 7th day postoperation, the concentration of serum albumin, prealbumin and transferrin were measured, and the body mass index (BMI) was calculated. The time of anal exhaust and diarrhea were observed and recorded. Meanwhile, the negative nitrogen balance was calculated everyday during perioperation. Results: The prealbumin concentration was significantly higher, but the negative nitrogen balance and the number and frequency of diarrhea were significantly lower in Nutrison MCT group than in Nutrison group on the 7th day postoperation (P 〈 0.05 ). There were no significant difference between two groups on the other indices. Conclusion : As a kind of special enteral nutrition used for fat malabsorption, the Nutrison MCT can improve the intestinal tolerance of early postoperative enteral nutrition, and thereby improve the nutritional condi- tion of patients postoperation.
