摘要
目的探讨不同阶段哮喘患者血清C反应蛋白(CRP)和肺功能测定的临床意义。方法回顾性分析了2009年1月到2010年10月期间,我院收治的120例哮喘患者和25例健康体检者为研究对象,观察哮喘不同时期及对照组的血清CRP、肺功相关指标(第一秒用力呼气容积占预计值的百分数(FEV1%)、1秒钟用力呼气容积与用力肺活量比值(FEV1/FVC)、氧分压(PaO2)及二氧化碳分压(PaCO2))以及外周血白细胞计数(WBC)。进一步观察不同病情哮喘患者血清CRP和外周血白细胞计数(WBC)的变化。结果与对照组比较,各个哮喘组CRP明显升高,差异显著,有统计学意义(P<0.05)。与慢性持续期、缓解期和对照组比较,急性发作期哮喘组血清CRP、WBC及PaCO2均明显增高,急性发作期哮喘组FEV1%、FEV1/FVC%及PaO2均明显下降、差异显著,有统计学意义(P<0.05)。不同病情哮喘患者,合并肺炎支原体,衣原体感染者、合并细菌感染者血清CRP、WBC明显高于单纯哮喘者,互相比较,差异显著,有统计学意义(P<0.05);其中合并细菌感染者血清CRP表达最高。结论血清CRP结合肺功能检测可作为判断哮喘患者病情的一个比较重要指标,有助于哮的诊断,值得推广。
Objective To discuss the clinical significance of inspection of C-reactive protein and pulmonary function in different stages.Methods The clinical information of 120 asthma patients and 25 health people from Jan.2009 to Oct.2010 were retrospectively analyzed.Serum CRP and pulmonary function indexes(such as FEV1%,FEV1/FVC,PaO2,PaCO2 and WBC) in different stages were observed.The change of serum CRP and WBC in different stages were further observed.Results Compared with control group,CRP in each asthma group were obviously increased,the difference was significant and had statistical significance(P0.05).Compared with persistent,remission stage and control group,CRP,WBC and PaCO2 of acute attack stage were obviously improved.FEV1%,FEV1/FVC% and PaO2 in acute attack stage were obviously decreased,the difference was significant and had statistical significance(P0.05).For patients who combined with mycoplasma pneumoniae,trachomatis infection and bacterial infection,CRP and WBC were obviously higher than those patients with simple asthma,the difference was significant and had statistical significance(P0.05).Among them,patients who combined with bacterial infection had the highest expression of serum CRP.Conclusion Serum CRP combined with pulmonary function can be considered as an important index for the evaluation of the state of asthma.It is helpful to the diagnosis of asthma and worthy of spreading in clinic.
