应用多普勒超声研究颈动脉病变对全脑血流量的影响 被引量:10

The Influence of Carotid Artery Disease on Global Cerebral Blood Flow Volume by Doppler Ultrasound

摘要 目的研究颈动脉狭窄程度对全脑血流量(CBFV)的影响,寻找早期诊断缺血性脑血管病的超声指标。方法选取69例接受颈部血管彩超检查的缺血性脑血管病患者,应用彩色多普勒超声技术测量其两侧颈内动脉、椎动脉内径(D)、狭窄程度及时间平均血流速度(TAMV),计算CBFV,比较不同程度颈动脉狭窄患者的CBFV。结果不同程度颈动脉狭窄患者的CBFV间差异有统计学意义(F=54.81,P<0.05);颈内动脉狭窄>70%组、狭窄50%~70%组患者的CBFV低于颈内动脉狭窄<50%组患者,差异均有统计学意义(q值分别为3.43和2.69,P<0.05),颈内动脉狭窄50%~70%患者的CBFV与颈内动脉狭窄>70%患者比较,差异无统计学意义(q=1.27,P>0.05)。结论经多普勒超声证实颈内动脉狭窄>50%的患者CBFV显著减少。 Objective To study the influence of the extent of carotid artery stenosis on global cerebral blood flow (CBFV), and to discover the early diagnostic ultrasound indexes of ischemic cerebrovascular disease. Methods 69 patients with ischemic cerebrovascular disease having received the neck vascular ultrasound test were chosen randomly as the subjects,whose internal carotid arteries and the vertebral arteries as well as the vertebral artery diameter ( D), the degree of carotid artery stenosis, and the Angle -corrected time -averaged flow velocity (TAMV) of the vessel were measured with the help of carotid color duplex. CBFVs were calculated and compared among the patients with different degrees of carotid artery stenosis. Results The difference of CBFVs among the patients suffering from different degrees of carotid artery stenosis was statistically significant (F=54. 81, P 〈0. 05). Compared with patients having lower than 50% of internal carotid stenosis, CBFV decreased signifi- cantly in patients having more than 70% of internal carotid stenosis and patients having 50% ~ 70% of internal carotid stenosis (q = 3.43, 2.69, P 〈 O. 05) ; There was no significant difference in CBFV between patients having more than 70% of internal carotid stenosis and patients having 50% - 70% of internal carotid (q = 1.27, P 〉0. 05 ). Conclusion CBFV decreased significantly in patients having more than 50% of internal carotid stenosis, which was confirmed by carotid color duplex.
