应用定量组织速度成像技术评价高血压患者左心房机械功能 被引量:4

摘要 目的:探讨应用定量组织速度成像(QTVI)技术检测高血压患者左心房局部心肌舒缩时的机械运动特性。方法:对38例正常对照组及41例高血压患者行彩色多普勒超声心动图检查,采集左心房侧壁、房间隔壁基底段及中段心肌的组织多普勒二维图像,定量分析上述各节段心房心肌运动的速度曲线及心房壁心肌收缩的起始时间、达峰时间与结束时间。结果:(1)与正常对照组比较,高血压组相同房壁、相同节段心室收缩期心房峰值速度(VS)、心房收缩起始时间(PA-T1)和达峰时间(PA-T2)差异无统计学意义(P>0.05),心室舒张早期心房峰值速度(VE)及VE/VA值减低,心室舒张晚期心房峰值速度(VA)增加,心房收缩结束时间(PA-T3)、收缩加速时间(△T2-T1)、减速时间(△T3-T2)及整个收缩时间(△T3-T1)延长,差异有统计学意义(P<0.05或P<0.01)。(2)高血压组相同房壁、不同节段的VS、VE、VE/VA值及心肌收缩时间各项指标差异无统计学意义(P>0.05),VA差异有统计学意义(P<0.05或P<0.01)。结论:高血压患者左心房心肌收缩速度增加,收缩加速时间、减速时间及整个收缩时间延长;QTVI技术可定量、简便、无创评价高血压患者左心房局部的机械运动特性。 Objective: To determine the feasibility of quantitative tissue velocity imaging(QTVI) to evaluate left atrial regional mechanical function in the patients with hypertension.Methods: In 38 cases of control group and 41 patients with hypertension,the basal and mid levels of left atrial lateral wall and inter-atrial septum were studied using QTVI.The parameters of the left atrial velocity and systolic time were analyzed.Results: Compared with the control group,some parameters such as the velocity in ventricular systolic time(V_S),the onset time and the peak time of left atrial had no significant differences,velocity in early ventricular diastolic time(V_E),V_E/V_A ratio of left atrial were reduced and the velocity(V_A),the end time,the accelerate time,the decelerate time,the whole systolic time of left atrial in ventricular late diastolic time were increased(P0.05 or P0.01).Conclusion: The systolic velocity,the accelerate time,the decelerate time and the whole systolic time of left atrial increased in patients with hypertension.QTVI may provide a quantitative,simple and noninvasive useful tool for left atrial regional function evaluation.
