改良式Forsus矫治器对安氏Ⅱ类错下颌后缩患者颞下颌关节影响分析 被引量:2

A magnetic resonance imaging analysis of temporomandibular joint changes of patients with Class Ⅱmandibular retrusion treated with modified Forsus appliance

摘要 目的研究改良式Forsus矫治器对安氏Ⅱ类错下颌后缩患者颞下颌关节的影响。方法选择2009—2011年来山东大学口腔医院正畸科就诊的采用改良式Forsus矫治器治疗的安氏Ⅱ类下颌后缩患者20例,应用改良式Forsus矫治器进行功能性矫形治疗,并对治疗前后颞下颌关节变化进行测量分析。结果改良式Forsus矫治器治疗6~8个月后,髁状突后上方出现增生迹象。髁状突与关节窝的位置关系、关节盘与关节窝的位置关系治疗前后差异均有统计学意义(P<0.05);而髁状突与关节盘位置关系治疗前后差异无统计学意义(P>0.05)。结论改良式Forsus矫治器治疗安氏Ⅱ类错过程中,可刺激髁状突、关节盘产生适应性改建,髁状突、关节盘、关节窝的相对位置关系可保持相互协调。 Objective To compare the temporomandibular joint changes on the magnetic resonance images before and after the treatment for Class II mandibular retrusion by using modified Forsus appliance, and to investigate the growth reconstruction of temporomandibular joint. Methods Twenty children with mandibular retrusion were selected and treated with modified Forsus appliance. Magnetic resonance images were taken and analyzed at pre-treatment and post-treat- ment. Results After 6-8 months of therapy, the signs of condylar muhiplication were observed at the posterior-superior border. The relationship between condylar and fossa and the relationship between disc and fossa had significant difference between pre-treatment and post-treatment. But the relationship between condylar and disc had no significant difference between pre-treatment and post-treatment. Conclusion In the progress of using modified Forsus appliant,condylar and disc produce adaptive reconstruction, and the relative position of condylar and fossa-disc can keep coordination.
