自然状态下非球面人工晶状体眼视觉质量的临床研究 被引量:2

Visual quality in patients with aspherical lens implantation: A clinical study under natural condition

摘要 目的 评价非球面人工晶状体眼的视觉质量。方法收集2008年10月至2009年3月行白内障超声乳化吸除并人工晶状体植入术的患者共32例(44只眼)。实验组17例(22只眼)植入Bausch&LombAkreosAO非球面人工晶状体,对照组15例(22只眼)植入Bausch&LombAkreosAdapt球面人工晶状体,随访时间为术后3个月,在暗室自然状态下,使用MFVA-100型多功能电子视力测量仪测量对比度视力,使用OPTEC6500视功能测试系统检测昼照明、夜照明、夜有眩光环境下五个空间频率的对比敏感度,使用iTrace视觉功能分析仪检测瞳孔直径和波前像差。结果100%、25%、10%、5%远距对比度视力,实验组与对照组视力差异无统计学意义;昼照明18cpd空间频率和夜照明6cpd、12cpd、18cpd空间频率的对比敏感度,实验组高于对照组,差异有统计学意义;44只眼平均瞳孔直径为(5.52±0.81)mm,其中82%的瞳孔直径大于等于5mm;实验组球差和彗差小于对照组,差异有统计学意义。结论Bausch&LombAkreosAO非球面人工晶状体能够降低患者术后的球差及彗差,能够提高对比敏感度,改善视觉质量。 Objective To evaluate visual quality in patients with aspherical lens implantation. Methods A retrospective study, which included 32 patients (44 eyes) who had accepted cataract phacoemulsification and intraocular lens implantation. Seventeen Patients (22 eyes) in the experimental group were implanted aspheric lens (Bausch & Lomb Akreos AO), while 15 Patients (22 eyes) in the control group were implanted spherical lens (Bausch & Lomb Akreos Adapt). Under natural scotopic condition, the contrast visual acuity, the contrast sensitivity, the pupil diameter and the higher order aberration were measured by the MFVA-100 multi-fimctional electronic vision measuring instrument, the OPTEC 6500 functional test system and iTrace visual function analyzer, respectively, at 3 months after surgery. Results The difference of 100%, 25%, 10% and 5% contrast visual acuity between the experimental group and control group was non-statistically significant. The contrast sensitivity in 18cpd spatial frequency under daytime lighting environment and in 6cpd, 12cpd, 18cpd spatial frequency under night lighting environment, was improved significantly in the experimental group. The average pupil diameter of the total 44 eyes was 5.52+ 0.81ram, and the pupil diameter in 82% of the eyes was or greater than 5mm. The spherical aberration and coma of the experimental group were lower than the control group, and the difference was statistically significant. Conelusions Bausch & Lomb Akreos AO aspheric lens can reduce spherical aberration and coma, and can improve contrast sensitivi- ty under the natural scotopic condition.
