摘要
世界发达国家出院计划发展较早,且已形成较科学、完善的标准,成为提供连续性健康照护的关键,是连续性护理得以延续的重要机制。文章介绍了英、美两国出院计划的概念、发展背景、现状,并总结了对我国开展出院计划的几点启示。
The discharge planning in the developed countries is scientific and systemic because of their long-time development.They played a key role in providing a consecutive health care.This paper summarized the concept,background and current status of discharge planning in Britain and American and raised some proposals for improving its development in China.
