摘要
目的观察连续硬膜外阻滞与气管插管静吸复合全麻在妇科腹腔镜手术麻醉中的应用，比较其优缺点。方法将120例妇科腹腔镜手术的麻醉随机分为持续硬膜外阻滞组（c组，60例）与气管插管全麻组（G组，60例），观察患者术中心率、血压、SP02、Pnc02变化，并记录手术时间、气腹时间及苏醒时间。结果两组患者术前、手术开始即褒4、气腹时、气腹后5min、放气及术毕时，心率、血压、SP02、PETCO2变化差异组间比较无统计学意义（P〉0.05）。两组的手术时间、气腹时阃变化无明显差异（P〉0．05），但术后苏醒时间比较，G组与C组相比差异有统计学意义（P〈0.01）。结论持续硬膜外阻滞与气管插管静吸复合全麻均可用于妇科腹腔镜手术．但在强化麻醉管理、充分吸氧的前提下，持续硬膜外阻滞应用于ASAⅠ－Ⅱ级级患者施行妇科腹腔镜短小手术是安全、可行、经济、实用的麻醉方法。
Objective Observe the continuous epidural block and endotracheal intubation inhalation anesthesia in gynecological laparoscopic surgery Anesthesia, compare their advantages and disadvantages. Methods 120 patients with gynecologic laparoscopic surgery were randomly divided .into continuous epidural anesthesia group（C group, 60 patients） and endotracheal intubatian group （G group, 60 patients）, observation of patients undergoing heart rate, blood pressure, SPO2, Pe7CO2 changes, and record the time of surgery, pneumoperitoneum time and recovery time. Results Two groups of patients before surgery began immediately, pneumoperitoneum, pneumoperitaneum after 5min, deflated and surgery, the heart rate, blood pressure, SPO2, PETCO2 change difference between the two groups are not significant （P〉0.05）. Two hours of surgery, pneumoperitoneum time are not significant difference （P〉0.05）, but postoperative recovery time compared, G group compared with the C group is significantly （P〈0.01）. Conclusion Continuous epidural and endotracheal intubation, inhalation anesthesia can be used for laparoseopic surgery, but in strengthening the anesthetic management, and fully under the premise of oxygen, continuous epidural used in ASA I - ]I grade level in patients implementation of short laparoscopic surgery is safe, feasible, economical and praetieal method of anesthesia.
