摘要
目的比较铸造金属桩核和纤维桩核修复上颌前牙残根的远期临床效果。方法本研究的对象为61名患者,共有171颗上颌前牙残根需要进行修复,其中115颗采用铸造金属桩核,56颗采用纤维桩核修复,所有牙齿最后均采用金属烤瓷全冠恢复外形。患者分别于6、18、36、48、60个月召回,拍摄X线牙片并结合临床检查,判定有无修复体脱落、根尖周病变、根折以及桩折,统计各种情况发生的例数。结果两组患者在根折发生率方面差异有统计学意义(P=0.039),而修复体脱落率(P=0.217)、根尖周病变发生率、桩折断发生率(P=0.154),两组患者间差异无统计学意义。在本研究所设定的时间范围内,两组修复体成功率之间的差异也无统计学意义(P=0.471)。结论纤维桩核可以很好地替代铸造金属桩核应用于口腔临床,其可以有效地减少根折的发生。
Objective To compare long-term clinical effects of residual root of maxillary anterior tooth restored with cast metal post and core and fiber post and core.Methods 61 patients with 171 endodontically treated residual roots of maxillary anterior tooth.Among them,115 residual roots were restored with cast metal post and core,the others were restored with fiber post and core.After 6 months,18 months,36 months,48 months and 60 months,patients were recalled,and a clinical and radiographic examination was performed to determine if there was restoration dislocation,Periapicallesion,root fracture or post fracture occured.Results There was significant difference for root fracture between two groups,but there was no significant difference for restoration dislocation and eriapicallesion and post fracture between two groups.Within the set range of time in this study,there was also no significant difference for the repaired tooth root survival rate between two groups.Conclusion Fiber post and core is an excellent alternative of cast metal post and core,it can reduce the occurrence of root fracture effectively.
