血管束植入神经移植段修复周围神经缺损 被引量:3

摘要 观察血管束植入神经移植段重建移植神经段血供的再血管化过程以及与神经再生的关系.方法:用血管束植入和不植入血管束神经移植段修复兔25mm坐骨神经缺损,采用血管造影,墨汁灌注,电生理检测,光镜,透射电镜,图像分析观察血管束植入后,神经移植段的再血管化过程,神经纤维再生及功能恢复.结果:血管束植入术后3d即有再生血管长入神经段外膜及柬间,3Wb移植神经段再血管化过程完成.而不植入血管束移植神经段再血管化过程45d才能完成,4Wb实验组移植神经再生纤维数较多,雪旺细胞增生明显.排列密集而规则,髓鞘再4wb已明显可见,3mo时其形态已完全恢复正常,而对照组则再生神经纤维数,雪旺细胞增生较少,排列疏散零乱,再生髓鞘极为罕见,其3mo时其形态仍差于实验组.结论:血管束植入可早期重建移植神经段血供,对神经的再生和功能恢复有明显的促进作用. AIM: To observe the revascularization and regen-eration of free nerves after vascular bundle implantation in re-pairing peripheral nerve defect. METHODS: Using freenerves with vascular bundle implantation to repair 2. 5 cmrabbit ischiadicus nerve defect and nerve samples were ob-tained at different times after the implantation while the con-trol group was treated with no vascular bundle implantation.The samples were studied with vascular radiography, ink irri-gation, electrophysiology, light microscopy, e1ectron andmotomatic image ana1ysis to observe the process of revascu-larization, nerve fiber regeneration and function restoration.RESULTS: Revascularization in outer membrane and nervebundle was observed 3 days after the implantation, and theprocess of revascularization completed within 3 weeks,whereas the same process in the control group did not com-plete until 45 days. As for the nerve regeneration, greatnumber of nerve fibers and Schwann's cel1s with regular ar-rangement and regenerated myelin sheath were observed 4weeks after the implantation. 3 months after the implanta-tion, free nerves with vascular bundle implantation regener-ated and restored to normal, while in the control group, onlya small number of nerve fibers and Schwann's cells with ir-regular arrangement and rare regenerated myelin sheath wereobserved. By the end of 3 months, the free nerve with novascular bundle implantation did not restore as well as thatwith vascular bundle implantation. CoNCLUS1ON: Vascularbundle implantation may help free nerves to restore bloodsupply and accelerate nerve regeneration and function restora-tion.
