摘要
目的 观察参麦注射液在急性心肌梗塞(AM I) 尿激酶溶栓治疗中抗心肌挫抑(M S)的临床疗效。方法 502 例 AM I患者随机分为两组,参麦组(243 例)采用参麦注射液和溶栓疗法治疗,对照组(259 例)只使用溶栓疗法。比较观察两组患者的梗塞相关血管(IRA)再通率、5 周病死率、副作用、各种严重并发症发生率、左室射血分数(EF)和CK、CK-M B 峰值变化。结果 参麦组IRA 再通率(78.3% )、出血( 轻度和中重度)发生率(17.28% )和 5 周病死率(3.29% )差异均无显著性(P 均> 0.05)。参麦组EF≥50% 者比率(62.55% )显著高于对照组(56.37% ,P< 0.01)。各种严重并发症发生率均显著低于对照组(P 均< 0.01)。参麦组CK 峰值(1252.21±285.11)和CK-M B 峰值(141.23±21.13)均显著低于对照组(分别为 1721.14±271.15,163.32±19.22,P< 0.01)。结论 尿激酶溶栓治疗AM I早期使用参麦注射液可促进心功能恢复,对因AM I心肌缺血再灌注造成的M S 具明显的对抗作用。
Objective To obseve the therapeutic effect Shenmai injection against myocardium stunning (MS)in thrombolytic therapy with urokinase for acute myocardial infarction(AMI). Method Five hundred and two cases of AMI were divided into two groups randomly: Shenmai group (243 cases) treated with thrombolytic therapy and Shenmai injection, control group( 259 cases) only with thrombolytic therapy. The reperfusion rate of infarct related coronary atery(IRA),side effects,morality, frequencies of various severe complications ,injection fraction (EF) and the peaks of CK and CK MB were compared between Shenmai group and control group. Result The reperfusion rate of IRA,the morality in first weeks and the rate of bleeding (minor and majoi) were not signficantly different between Shenmai group and control group(78.61% vs 78.38%,3.29%vs 3.47%,17.28vs17.37%,P all>0.05). The rate of EF≥50% was higher than that of control group (62.55% vs 56.37%,P<0.01). Frequencies of various severe complications in Shenmai group were lower than those of control group (P all< 0. 01) . The peaks of CK and CK MB in Shenmai group were significantly decreased (1252.21±285. 11 vs 1721.14±271.15,P<0.001;141.23±21.13 vs 163±19.22, P<0.01)。Conclusion Shenmai injection can enhance the heart function,decrease frequencies of various severe complications and level of the peaks of CK and CK MB in thrombolytic therapy with UK for AMI. So Shenmai injection can improve MS caused by ischemic reperfusion in AMI.
