白血病多药耐药基因表达检测及其意义

摘要 目的:研究多药耐药基因在白血病中表达的情况及意义。方法:应用逆转录—多聚酶链反应(RT- PCR)和免疫细胞化学APAAP 方法检测了50 例白血病患者多药耐药( mdr - 1) 基因及P- 糖蛋白(Pgp) 的表达情况并进行分析研究。结果: mdr- 1 及Pgp 的表达与患者的年龄有关, 50 岁以上患者的阳性率显著增高( P< 0.05, P<0 .01) 。未经治疗的初诊患者mdr- 1 的阳性率显著低于反复化疗的患者( P< 0 .05) 。Pgp 表达与临床疗效的总符合率为72 .5 % , 低于m dr - 1 的表达与临床疗效的总符合率86 % 。两项指标联合检测, 其临床疗效总符合率最佳91.3 % 。结论:m dr - 1 及Pgp 是两个良好的临床多药耐药监测指标,尤以二者联合应用为佳。 Objective: To study the expression of multidrug resistance gene in leukemia and its clinical implication. Methods: Expression of multidrug resistance (mdr-1) gene and P-glycoprotein (Pgp) were determined in 50 cases of leukemia by RT-PCR and immunocytochemical APAAP assays. Results: The results showed that expressions of mdr-1 and Pgp were related with patient′s age. The mdr-1 and Pgp expressions were significantly higher in patients over 50 years (P< 0.05, P< 0.01). The positive rate of mdr-1 in patients received repeated chemotherapy were higher than those of the patients untreated with chemotherapy (P< 0.05). The coincident rate of Pgp and clinical curative effect was 72.5% which was lower than that between mdr-1 and clinical curative effect (86%). Determination of mdr-1 and Pgp at the same time was more valuable to predict the clinical curative effects of leukemia (91.30%). Conclusion: Both mdr-1 and Pgp expressions are good indicators of MDR in clinic especially when assayed simultaneously.
