摘要
目的：分析后路腰椎间盘摘除术椎管内出血的原因及减少出血的方法。方法：在２１ 具成人脊柱腰段标本和１００ 例青壮年ＣＴ片上对椎内静脉干进行断层解剖及ＣＴ影像观测。测定２３ 例后路腰椎间盘摘除术术中出血等项目。结果：在硬膜囊与椎体、椎间盘之间，后纵韧带两侧各有１ 条粗大的静脉干，外径平均为３．７ ｍ ｍ ，ＣＴ片上口径２．６ ｍ ｍ 。经２３ 例临床应用，首先结扎椎内静脉干，测定其出血量，平均（１８１．０４±５４．７４）ｍ ｌ。结论：在椎内静脉前丛内存在两条纵行的静脉干。
Objective:To analyze the causes of bleeding and the methods of bleeding decrease in vertebral canal during the posterior approach discectomy.Methods:The internal vertebral venous trunk was observed on the lumbar region of 21 adult cadavers and 100 adult CT images.Bleeding and other items were measured during 23 posterior approach discectomies.Results:There was a thick venous trunk on each side of the posterior longitudinal ligament and between dural capsule and vertebral body.The average diameter of the internal vertebral venous trunk was about 3.7 mm on the cadavers and about 2.6 mm on the CT image.After clinical practice in 23 patients,the average bleeding was 181.04±54.74 ml,by ligaturing first the internal vertebral venous trunk. Conclusion:There are two vertical venous trunk in the anterior internal vertebral plexus.Bleeding can be significantly decreased by ligaturing first the internal vertebral venous trunk during the posterior approach discectomy. KEY WORDS discectomy;internal vertebral venous trunk;hemostasis. used for treating 30 cases of diabetic peripheral neuropathy.7 cases were obviously effective,21 cases were effective while 2 cases ineffective.The total effective rate was 93.3%.The result showed that the therapy could effectively relieve the subjective symptoms and sings,improve the nerve conductive velocity and promote the recovery of nerve function.
