视频脑电图与头颅影像学在小儿癫痫诊断中的作用 被引量:4

Correlative relationship between video electroencephalogram and brain imaging in the diagnosis of children epilepsy

摘要 目的观察视频脑电图(VEEG)和头颅影像学检查在小儿癫痫及其病因诊断中的作用。方法对154例癫痫患儿的常规脑电图(REEG)、VEEG、头颅MRI和CT检查结果进行回顾性分析。结果 154例中55例(35.7%)患儿常规EEG可见癫痫性放电;119例(77.2%)患儿VEEG可见癫痫性放电;96例(62.3%)患儿头颅MRI检查发现病灶,52例(33.8%)头颅CT检查发现病灶。局灶放电和弥漫放电患儿头颅MRI局灶异常率差异有显著统计学意义(<0.01)。颞叶和颞叶外放电定位符合率差异有显著统计学意义(<0.01)。结论 VEEG发现癫痫性放电的异常率明显高于REEG,MRI可显示与癫痫性放电病灶有关的结构特征,是癫痫病因诊断重要的定性手段。电生理检查发现的癫痫性放电部位与神经影像学检查显示的病灶,特别是颞叶病灶具有显著的关系。 Objective To analyse the role of video electroencephalogram(VEEG) and magnetic resonance imaging(MRI)indiagnosisofepilepsyonitsprimary epilepticfocusandpathologiclesions.Methods Aretrospectiveanalysis was conducted on 154 epileptic children with datum of routine EEG,VEEG,MRI and CT scanning.Results Compared with the rate of 35.7% in routine EEG,VEEG showed a higher sensitivity in finding epileptic focus in 77.2% of patients.62.3% of patients were found cerebral lesions by MRI,and 33.8% by CT scan.Discharging foci of VEEG corresponded to lesions by MRI.MRI showed more primary lesions accordant with epileptic discharge in patients with temporal dis-charge than those with out of temporal discharge(P〈0.05.) Conclusions The abnormal rate of VEEG in finding epile-ptic discharge was higher than that of routine EEG.MRI is an important assistant method to illustrate the pathologic char-acter of lesions.There is a correlative relation between electrophysiologic findings and neuroimaging results,especially in temporal lobe.
