喉罩联合靶控输注在老年重症患者手术麻醉中的临床应用 被引量:1

摘要 目的评价喉罩及靶控输注丙泊酚瑞芬太尼用于老年重症病人短小手术的使用效果。方法 42例非口内和胸腹手术老年危重病人随机分为两组,Ⅰ组为喉罩组,Ⅱ组为普通气管插管组,记录诱导前、置入喉罩、切皮、术中最低值、清醒拔出喉罩气管导管后五分钟的BIS值、平均动脉压、心率变化和血气分析。结果喉罩组循环动力学稳定,术后恢复快,安全性高。结论靶控输注丙泊酚和瑞芬太尼时喉罩通气是高龄重危病人麻醉的理想选择。 【Objective】 To evaluate the security of laryngeal mask airway(LMA) and target controlled infusion(TCI,remifentanil and propofol) to the elder in the transitory operation.【Methods】 Forty-two surgical elderly patients,had out of oral、chest and stomach operation,were selected,graded I-Ⅲof American Society of Anesthesiologjsts(ASA).They were randomly divided into laryngeal group(I-group)and intubate group(II-group),with 21 cases each.HR、BP、MAP and BIS were monitored continuously in two groups,the data of blood gas analysis were recorded pre-anesthetized、during operation and five minutes after recovery.【Results】 Every operation was accomplished without any intraoperative consciousness.In groupⅠ,the extent of MAP and HR is narrow during the operation.On the contrary,patients in groupⅡhad larger fluctuation of HR and MAP,longer recovery time.【Conclusion】 Laryngealmask airway with TCI of Propofol and Remifentanil to the elder can get satisfied depth of anesthesia,and the recovery is better.So it is better than that by general intubate anesthesia.
