摘要
目的探讨经直肠腔内超声（TRUS）引导下经会阴前列腺穿刺活检术中应用针刺辅助镇痛的临床价值。方法将105例拟行直肠腔内超声引导10点前列腺系统穿刺活检患者采用随机数字表法分为两组，A组54例采取单纯前列腺周围神经阻滞，B组51例采取前列腺周围神经阻滞加针刺双侧足三里。采用视觉模拟评分（VAS）评估患者在穿刺活检术中的疼痛程度。结果A、B两组患者平均疼痛评分（VAS）分别为（2．8±1．0）、（0．9±0．8）分，差异有统计学意义（t=10．71，P〈0．01）。两组患者均未出现局麻药或针刺的不良反应。结论针刺辅助镇痛可明显减轻患者在TRUS引导下前列腺穿刺活检术中的疼痛和不适，是一种更加简便、有效的复合麻醉方法。
Objective To assess the value of acupuncture-assisted analgesia for transrectat-ultrasound-guided biopsy of the prostate by transperineal approach. Methods One hundred and five patients requiring 10 cores biopsy of the prostate were randomly assigned into two groups. Group A （54 cases） received the periprostatic nerve plexus block, and group B （51 cases） received the periprostatic nerve plexus block and acupuncture at Zusanli point. Visual Analogue Score （VAS） was used to evaluate the pain extent during the procedure. Results The mean pain scores during transperineal prostate biopsy were significantly lower in group B than those in group A （0.9 ± 0.8 vs 2.8 ± 1.0, t=10.71, p〈 0. 01）. No patients in either group showed any adverse effects from the injection or acupuncture. Conclusion Acupuncture-assisted anesthesia is a safe and effective analgesic method for transperineal prostate biopsy guided by transrectal ultrasonography.
