摘要
目的观察中西医结合治疗肾功能正常IgA肾病的临床疗效。方法 60例确诊为IgA肾病患者随机分为治疗组(30例)和对照组(30例)。对照组以单纯西药治疗,治疗组在西药的基础上,通过中医辨证分型论治,口服中药汤剂治疗,3个月为1疗程,观察两组治疗前后临床症状、体征、尿红细胞计数、24 h尿蛋白定量及肾功能等指标的变化。结果治疗组完全缓解9例,基本缓解8例,好转11例,无效2例,总有效率93.33%;对照组完全缓解3例,基本缓解4例,好转10例,无效13例,总有效率56.67%。两组总有效率有显著性差异(P<0.05)。治疗组在治疗后24 h尿蛋白定量、尿RBC计数明显减少,与治疗前比较,有非常显著差异(P<0.001)。治疗组较对照组明显减少24 h尿蛋白定量(P<0.01),而在减少尿RBC计数方面治疗组也优于对照组,有非常显著性差异(P<0.001)。结论中西医结合治疗IgA肾病在减轻临床症状、减少血尿、蛋白尿,减轻激素副作用等方面均有较好疗效。
Objective To observe the clinical effect of Integration of traditional Chinese and western medicine for the treatment of IgA nephropathy with normal renal function.Methods 60 patients diagnosed as IgA nephropathy were randomly divided into therapeutic group（30 cases） and control group（30 cases）.The control group was treated with western medicine alone,and the therapeutic group was given oral TCM decoction based on syndromes differentiation besides western medicine treatment.3 months was a treatment course.The changes of clinical symptoms,physical signs,urine RBC count,24h urine protein and renal function of the two groups after the treatment were observed.Results the therapeutic group： 9 cases were completely relieved,8 cases were relieved,11 cases got better,2 cases were ineffective,and the total effective rate was 93.33%.The control group： 3 cases were completely relieved,4 cases were relieved,10 cases got better,13 cases were ineffective,and the total effective rate was 56.67%.The effective rates of the two groups were significantly different（P〈0.05）.After the treatment,24h urine protein,urine RBC count of the therapeutic group reduced obviously.There was a great difference compared the condition before the treatment（P〈0.001）.Compared with the control group,24h urine protein of the therapeutic group decreased markedly（P〈0.01）.The therapeutic group also had more reduction on urine RBC count than the control group,and the difference was significant（P〈0.01）.Conclusion Integration of traditional Chinese and western medicine has good effects for the treatment of IgA nephropathy on the relieving of clinical symptoms,the reduction of blood urine and proteinuria and the lessening of side effects of hormones.
