The level of brain natriuretic peptide and prognosis in severe cerebral infarction patients

摘要 目的探讨重症脑梗死患者血浆脑钠肽(brain natriuretic peptide,BNP)水平能否作为预测患者预后的指标。方法 60例急诊入住ICU的重症脑梗死患者入院时测定BNP水平,观察30d的病死情况,并将患者分为死亡组和存活组。比较两组之间入院时BNP水平的差异,同时采用Logistic回归分析筛选与死亡相关的因素。结果死亡组患者入院时BNP水平较存活组高,差异有统计学意义(P<0.05)。Logistic回归分析表明入院时BNP水平是增加患者死亡的危险因素。结论重症脑梗死患者血浆BNP水平是预测患者预后的指标之一,入院时BNP水平越高,死亡风险越大,预后越差。 Objective To investigate whether the plasma brain natriuretic peptide (BNP)level on admission can serve as a biological marker of prognosis in patients with severe cerebral infarction. Methods We prospectively enrolled 60 consecutive patients with acute severe cerebral infarction within 24 hours of onset and measured plasma BNP on admission. Patients were observed 30 days and divided into two groups : the deceased group who died during hospitalization and the survival group;The factors associated with in - hospital death were investigated by multivariate logistic regression analysis. Results The level of brain natriuretic peptide in plasma in the deceased group were significantly increased compared with the survival group. Multivariate logistic regression analysis demonstrated that the level of brain natriuretic peptide was independent factor associated with 30 d mortality. Conclusion The plasma BNP level on admission is one of the factors that can predict death risk in patients with severe cerebral infarction.
