食管鳞状上皮癌变过程中间质纤维母细胞的平滑肌肌动蛋白α和CD34表达及意义 被引量:3

摘要 目的观察食管鳞状上皮癌变过程中间质纤维母细胞的平滑肌肌动蛋白α(smooth muscle actin-α,αSMA)和CD34表达,探索肿瘤相关纤维母细胞(carcinoma-associated-fibroblasts,CAFs)在食管癌发生发展中的作用。方法采用免疫组织化学链霉素抗生物素过氧化酶连接(SP)法检测136例食管病变(正常20例、低级别上皮内瘤变26例、高级别上皮内瘤变44例、早期癌23例、进展期癌23例)间质组织中纤维母细胞αSMA和CD34的表达。结果食管鳞状上皮癌变过程中各组的间质纤维母细胞αSMA表达逐渐上升(0%,11.5%,40.9%,65.2%,95.7%);相应的CD34表达逐渐下降(95.0%,80.8%,50.1%,34.8%,0%)。高级别上皮内瘤变、早期癌组和进展期磷癌组的SMA和CD34与正常对照组比较差异有统计学意义(P<0.01)。结论食管鳞状上皮癌变过程中存在CAFs的转化形成,CAFs可能参与食管鳞状上皮的恶性转化及发展,可能是预测食管癌前病变转归的重要指标。 Objective To observe the expressions of smooth muscle actin-α(αSMA) and CD34 in the process of esophageal squamous epithelial canceration.To explore the role of carcinoma-associated-fibroblasts(CAFs) in esophageal cancer.Methods The SP method of immunohistochemistry was employed to measure the expressions of αSMA and CD34 in 136 specimens of esophageal tissue,including 20 specimens of normal esophageal tissue,26 specimens of low level intraepithelial neoplasia,44 specimens of high level intraepithelial neoplasia,23 specimens of early cancer,23 specimens of advanced cancer.Results There was ascendant trend in αSMA expression of stromal fibroblasts in the process of esophageal epithelial canceration(0%,11.5%,40.9%,65.2%,95.7%),and meanwhile descendant trend of CD34(95.0%,80.8%,50.1%,34.8%,0%).Significant difference was found in the positive expression of αSMA and CD34 in three neoplasia or cancer groups except the low level intraepithelial neoplasia compared with normal esophageal tissue(P〈0.01).Conclusion With the process of esophageal squamous epithelial canceration,there were stromal fibroblasts transforming into CAFs,suggesting that CAFs can induce esophageal squamous epithelial transforming and further developing,and could be good prognosis indicators in esophageal precancerosis.
