摘要
目的探讨SchatzkeⅣ、Ⅴ、Ⅵ型胫骨平台骨折手术内固定的临床疗效。方法对21例患者根据分型采用不同内固定方法：6例Ⅳ型骨折采用L型或T型支持钢板内固定,11例Ⅴ型骨折采用高尔夫（或锁定）钢板加拉力螺钉内固定,4例Ⅵ型骨折采用高尔夫（或锁定）钢板内固定。14例行自体髂骨植骨。结果术后有1例切口脂肪液化,无切口感染。21例均获随访,时间6~18个月。摄片复查均已骨性愈合。膝关节功能按Merchant评分标准：优4例,良14例,中3例。结论胫骨平台骨折按类型选择相应的内固定方法可达到较好的疗效。
Objective To investigate the clinical efficacy in Schatzke Ⅳ,Ⅴ,Ⅵ-type tibial plateau fractures treated by internal fixation.Methods According to fracture type,different kinds of internal fixation were used to treat 21 tibial plateau fracture patients： 6 cases of Ⅳ-type fracture were used L or T-supported plate,11 cases of Ⅴ-type fracture were used golf（or locking） Niagara force plate screw,and 4 cases of Ⅵ-type fracture were used golf（or locking） plate.Bone grafting was used according to bone defect in 14 cases.Results In one case,the wound got fat liquefaction without wound infection.21 cases were followed up for 6 to 18 months.In radiography review,the bone healed.Grading by Merchant rating criteria,knee function was excellent in 4 cases,good in 14,and fair in 3.Conclusions Based on the type,tibial plateau fracture can be treated by appropriate method of internal fixation with good therapeutic effect.
