带旋髂深动脉髂骨瓣与腓肠神经营养血管皮瓣修复跟骨及软组织缺损

摘要 目的探讨带旋髂深动脉髂骨瓣、腓肠神经营养血管皮瓣修复跟骨及软组织缺损的临床疗效。方法采用游离带旋髂深动脉髂骨瓣联合腓肠神经营养血管皮瓣修复6例跟骨及软组织缺损患者。结果 6例皮瓣骨瓣全部成活。3例皮瓣发生部分边缘性坏死,经换药后创面愈合;1例因跟腱修复失败,二期行踝关节融合术。患者均获得6~20个月随访。术后小腿及足部均有良好外形及部分感觉,部分患者经锻炼后恢复承重功能。结论以带旋髂深动脉髂骨瓣联合腓肠神经营养血管皮瓣修复跟骨及软组织缺损可行,临床疗效满意。 Objective To investigate the clinical results of vascularized iliac flap based on deep circumflex iliac artery combined with neurocutaneous flap in repairing soft tissue and bone defects of calcaneus.Methods Vascularized iliac bone incorporated with sural neurocutaneous flap were used to repair soft tissue and bone defects of calcaneus in 6 cases.Results The flaps survived completely in 6 cases,whereas 3 cases got marginal necrosis,and finally healed after dress changing.There was 1 case that received ankle arthroplasty because of failure grafting for Achilles′s tendon.All cases were followed up for 6 to 20 months.All cases got good appearance and partial sensation recovery,and some patients restored weight bearing with practice.Conclusions The application of vascularized iliac flap with deep circumflex iliac artery with neurocutaneous flap is quite easy and effective for calcaneous defect.
