摘要
目的:研究活血止痛软胶囊对骨折合并急性软组织损伤患者血液流变学及局部氧分压的影响,探讨其治疗骨折合并急性软组织损伤后改善局部肿胀及疼痛的作用机理。方法:收集我院2009年5月～2010年5月骨折合并急性软组织损伤住院病人112例。随机分为治疗组70例和对照组42例。治疗组常规给予口服活血止痛软胶囊,对照组给予安慰剂治疗。治疗期间停用其他治疗方法,观察治疗前后临床症状的改善情况。并于治疗前后检测各项血液流变学指标。应用中医氧分压传感针测定治疗前后局部组织氧分压的变化。结果:112例患者口服活血止痛软胶囊治疗后,患者临床症状明显改善。查体显示,软组织肿胀与疼痛均明显好转(P<0.01)。与治疗前相比,治疗组病人全血粘度及血浆粘度、胞聚集指数和刚性指数、血沉方程K值及纤维蛋白原均有所下降(P<0.05),变形指数增高(P<0.05),血球压积比较差异无统计学意义(P>0.05)。对照组患者上述指标比较差异均无统计学意义(P>0.05)。治疗前后局部氧分压治疗组差异有统计学意义(P<0.01),对照组差异无统计学意义(P>0.05)。结论:活血止痛软胶囊能通过改善局部微循环,增加局部氧供给,改善局部组织代谢达到早期活血散瘀,消肿止痛功能,疗效肯定。
Objective：To investigate the influence of Huoxue Zhitong Capsule（HZC） in hemorheology and partial pressure of oxygen in patients with fracture combined with acute soft tissue injury（STI）,and the mechanism involved.Methods：One hundred and twelve in-patients with fracture combined with STI from May 2009 to May 2010 were randomly assigned to treatment group（n=70） and control group（n=42）.HZC and placebo were respectively given to patients in the two groups,2 capsules once,and three times each day.The changes in clinical symptoms and indexes including hemorrheology and partial pressure of oxygen were observed.Results：The patients＇ symptoms in treatment group were significantly improved,with soft tissue swelling and pain obviously ameliorated（P0.01）.There were significant differences in indexes relating to hemorrheology in treatment group,such as blood viscosity,plasma viscosity,red cell aggregation index and rigid index,erythrocyte sedimentation rate equation quotient（K）,fibrinogen（P0.05）.No remarkable difference was found in control group in the above indexes（P0.05）.After the treatment,significant difference was observed in partial pressure of oxygen in treatment group compared with before the treatment（P0.01）,while there was insignificant difference in control group（P0.05）.Conclusion： HZC can improve regional microcirculation,increase local oxygen supply and improve tissue metabolism,thus applying the function of detumescence and relieving pain.
