带血管大鼠胸腺移植的应用解剖

摘要 目的明确大鼠胸腺解剖,设计带血管大鼠胸腺移植模型。方法对10只6~8周龄SD供体大鼠胸腺动脉的起源、静脉回流进行观察研究,设计带血管的大鼠单侧叶和双侧叶胸腺移植物血管蒂。15只6~8周龄SD受体大鼠分为左侧叶、右侧叶和双侧叶胸腺移植组,每组5只。观察胸腺移植物血供重建情况,并就供胸腺摘取方法、供吻合的血管蒂进行讨论。结果胸腺左侧叶动脉起源于发自左锁骨下动脉的左胸廓内动脉,左侧叶静脉回流至左胸廓内静脉、左无名静脉、后腔静脉。胸腺右侧叶动脉起源于发自右锁骨下动脉的右胸廓内动脉,右侧叶静脉回流至右胸廓内静脉、前腔静脉。所有受体开放血流后,胸腺移植物均得到良好血供。结论大鼠胸腺解剖有其自身特点,大鼠左、右单侧叶胸腺及双侧叶胸腺均可成功设计行单独带血管移植。 Objective To clarify the anatomy of rat thymus and design vascular thymus transplantation in rat.Methods The arterial origin and venous drainage of thymus in 6-8 weeks old SD donor rat(n=10) was studied.The vascular pedicles of unilateral or bilateral thymus grafts were designed.Recipient 6-8 weeks old SD rats were divided into three groups based on receiving a left lobe(n=5),right lobe(n=5) or both lobes(n=5) of thymus grafts.The blood supply reconstructions of the thymus grafts were evaluated in each group.The method of graft harvesting and pedicles retained in the grafts for anastomosis were analyzed.Results The right/left thymic lobe arteries originate from the right/left internal thoracic arteries which branched out from the right/left subclavian arteries.The venous drainage from the right thymic lobe originates from the right thymic lobe vein and drains into the right internal thoracic and anterior caval vein.The venous drainage from the left thymic lobe originates from the left thymic lobe vein and drains into the left internal thoracic,anonymous and finally posterior caval veins.The blood supplies of all grafts were well reconstructed after releasing vascular clamps.Conclusion Both unilateral and bilateral vascular thymus grafts can be designed for transplantation isolately in rat.
