摘要
目的探讨阿霉素肾病(ADN)大鼠肾组织nephrin及podocalyxin蛋白表达及泼尼松的干预作用。方法 24只雄性Wistar大鼠随机分成ADN组、泼尼松组、对照组。于0、4、8周测定大鼠24 h尿蛋白(24hUP)、血清胆固醇(Cho)及白蛋白(Alb)。实验8周取肾组织观察病理形态改变,计算肾小球细胞外基质面积与肾小球面积之比(ECM/GA),评估肾脏病理积分。用免疫组织化学法观察肾小球nephrin、podocalyxin表达。结果实验4周时,ADN组较之对照组24hUP、Cho升高(P<0.05),Alb下降(P<0.01)。实验8周时,泼尼松组较ADN组24hUP、Cho下降,ECM/GA及肾脏病理积分降低,nephrin、podocalyxin表达增多,Alb升高(P<0.01)。ADN组nephrin、podoca-lyxin表达与24hUP、Cho、ECM/GA及肾小球病理积分均呈负相关,与Alb呈正相关;nephrin与podocalyxin呈正相关。结论 nephrin、podocalyxin与蛋白尿的发生及发展密切相关。泼尼松能减轻ADN大鼠的蛋白尿及肾脏病理损害,其机制可能通过上调nephrin、podocalyxin表达来发挥肾保护作用。
Objective To investigate the expressiun of nephrin and podocalyxin in rats with adriamyein-induecd nephrosis (ADN) and the intervention effect uf prednisune. Methods Twenty-four male Wistar rats were randomly divided into three groups, which were ADN gr.up, prednisone group, eontrol group. Serum cholesterol, alhtunin (Alh) , and the 24-hour urine protein (24hUP) were measured serially in the 0, 4th, aud 8th week. Rats were sacrificed to collect kidney tissue for pathological mnrphology ohservatiun in the end of study ( the 8th week). Extracellular matrix/glomerular area ratio (FCM/GA) and renal pathology points were determined. The expression of nephriu anti poduealyxin in glomerulus were examined by means of immunohistnehemistry. Results In the 4th week, 24hUP and cholesterol were significantly higher in ADN group than those in control grnup respeetively (P 〈0.05) , while AIh was significantly lower (P 〈 0.01 ). In the 8th week, cumpared with ADN grnup, in prednisone group, 24hUP, eholesterol, ECM/GA, and renal pathology points were significantly lower (P 〈 0.01 ) , while Alh and the expression uf nephrin and poduealyxin were significantly higher (P 〈0.01 ). The expression of nephrin and pndocalyxin in ADN gruup were negatively currelated with 24hUP, cholesterol, ECM/GA and glomerular patholngy point, and positively eurrelated with Alb. The expression of nephrin was positively cor- related with that of podocalyxin. Conclusions Nephrin and podocalyxin are elusely related to the progression of pruteinuria. The mechanism of the prednisone alleviating proteinufia and kidney pathological damage may he enrrelated to the up-regulation of the expression of nephrin and podocalyxin.
