摘要
目的:探讨颈部淋巴结结核的临床特征、治疗方法及临床疗效。方法:将本院2008年6月～2009年5月收治的40例颈部淋巴结结核的患者随机分为观察组和对照组,每组20例,对照组采用抗结核药物治疗、手术治疗,观察组在对照组的基础上采用中药汤剂消瘰汤治疗,比较两组患者的临床疗效。结果:颈部淋巴结结核的CT表现为肿块呈均匀或不均匀强化,部分肿块呈融合现象。观察组、对照组的切口愈合率为95.0%、90.0%,差异无统计学意义,P>0.05。观察组的总有效率为100.0%,对照组为95.0%,差异无统计学意义,P>0.05。全部患者随访2年,观察组患者均无再发淋巴结肿大,对照组有2例复发,手术后治愈,复发率为10.0%,观察组显著优于对照组,P<0.05。结论:颈部淋巴结结核临床表现复杂,胸部X线与病理组织活检能够有效帮助其确诊,中西医结合治疗治愈率高,复发率低,且不良反应少,值得临床推广。
Objective：To investigate the clinical characteristics of cervical lymph node tuberculosis,treatment and clinical effect.Methods：From June 2008 to May 2009,40 cases with cervical lymph node tuberculosis were be treated in our hospital and they were divided into two groups that both had 20 patients randomly.In the control group we made use of the anti-TB drug therapyand surgery,the observation group used the decoction of Chinese medicine consumption which had been treated,then contrast the clinical efficacy of two groups.Results：The CT manifestations of cervical lymph node tuberculosis as a mass showed homogeneous or heterogeneous enhancement,and some parts of the mass showed a fusion phenomenon.The wound healing rate of observation group was 95.0% when it was 90.0% in control group,no significant difference,P〉0.05.The total efficiency of observation group was 100.0% and the control group was 95.0%,no significant difference,P〉0.05.All the patients were followed up for 2 years,there was no recurrence in patients with enlarged lymph nodes in the observation group,there were 2 cases of recurrence appeared in the control group and had been treated by surgery,the recurrence rate was 10.0% in the observation group.It means that the total efficiency of observation group was better than the control group,P〈0.05.Conclusion：The clinical manifestations of cervical lymph node tuberculosis is complex,and the chest X ray and pathological tissue biopsy is benefit to the diagnosis,the cure rate of is high,low recurrence rate and fewer side effects so the integrative medicine is worthy to expand.
