摘要
目的探讨抗房颤起搏器对病态窦房结综合征(SSS)伴阵发性房颤(PAF)患者的预防作用。方法 21例SSS伴PAF患者,7例置入Vitatron 900E型起搏器、1例置入Vitatron 9000型起搏器,13例置入T70A1DR起搏器,术后进行的定期随访,观察比较术后起搏监测阶段、起搏治疗阶段I及起搏治疗阶段II房颤事件。结果 21例患者起搏器置入术后起搏治疗阶段I与监测阶段比较,有症状PAF事件数(9.34±3.76和10.73±5.42)、PAF总数(126.2±84.9和136.9±98.2)、AF总持续时间[(9.39±3.82)d和(11.26±4.86)d]、AF负荷[(10.43±4.71)%和(12.52±8.76)%]差异均无统计学意义(均P>0.05);术后起搏治疗阶段II与监测阶段比较,有症状PAF事件数(5.07±2.32和10.73±5.42)、PAF总数(107.7±87.2和136.9±98.2)、AF总持续时间[(5.90±2.69)d和(11.26±4.86)d]、AF负荷[(6.56±3.22)%和(12.52±8.76)%]差异均有统计学意义(均P<0.05)。所有患者无血栓栓塞及死亡事件发生。结论抗房颤起搏器能减少SSS患者PAF的发生,降低房颤负荷,具有明显的近中期疗效。
Objective To evaluate the prevention effect of anti-atrial fibrillation pacemaker on paroxysmal atrial fib-rillation（PAF） in patients with sick sinus syndrome（SSS）.Methods Twenty-one patients with SSS were implanted with pacemakers,among these patients,seven cases with Vitatron 900E pacemakers,one case with 9000 pacemaker and 13 with Vitatron T70A1DR pacemakers.Regular postoperative follow-up was conducted and PAF episodes were surveyed in monitoring stage after pacing,stage I and stage II after cardiac pacing therapy.Results There were no significant differences between stage I after cardiac pacing therapy and monitoring stage on the number of sympto-matic PAF episodes（9.34±3.76 10.73±5.42）t,he total number of PAF episodes（126.2±84.9 136.9±98.2）,AF to-tal duration[（9.39±3.82）d（11.26±4.86） d] and AF load[（10.43±4.71）%（12.52±8.76）%]t;here were significant differences between stage II after cardiac pacing therapy and in monitoring stage on the number of symptomatic PAF episodes（5.07±2.32 10.73±5.42）t,he total number of PAF episodes（107.7±87.2 136.9±98.2）,AF total duration [（5.90±2.69）d（11.26±4.86）d] and AF load[（6.56±3.22）%（12.52±8.76）%].Death and thrombo-embolism didn＇t occur in all of the patients.Conclusions Anti-atrial fibrillation pacemaker can decrease occurrence of paroxysmal PAF and AF burden in patients with sick sinus syndrome,with a significant recent mid-term efficacy.
