摘要
目的总结肺泡蛋白沉着症在胸部CT、HRCT上的表现并分析误诊原因。方法回顾性分析我院5例经病理及临床证实的肺泡蛋白沉着症患者的胸部CT及HRCT征象。结果 PAP肺部影像表现呈多样化,其在CT及HRCT的主要表现可归纳为:地图样分布、肺实变样及铺路石样改变等。但有2例患者出现误诊。结论胸部CT及HRCT可以充分显示肺泡蛋白沉着症具有病变肺组织与正常肺组织截然分开、地图样改变及铺路石征等特点,结合其临床表现与影像学表现不平衡的临床背景可基本对本病作出正确诊断。
Objective To investigate the CT,HRCT features of the pulmonary alveolar proteinosis（PAP） and analysis the misdiagnosis.Methods The CT and HRCT performance of five PAP cases which had been confirmed by pathological and clinical examination were retrospective analyzed.Results PAPs pulmonary imaging findings were diverse,the main performance on CT and HRCT can be summarized as follows： the distributed geographically,airspace consolidation and crazy paving sign.Conclusion Pulmonary CT and HRCT can fully demonstrate the characteristics of PAP that lung lesions completely separate with normal lung tissue,distributed geographically and crazy paving sign.Combined with clinical background that the imaging manifestations were stabile and more serious than the clinical symptoms can lead us to give a correct diagnosis of PAP.
