改良穿针外固定架与DHS治疗高龄股骨粗隆间骨折分析 被引量:2

摘要 目的比较改良穿针单臂外固定架术与动力髋螺钉(dynamic hip screw,DHS)治疗高龄股骨粗隆间骨折的疗效。方法 80例股骨粗隆间骨折患者,分成单臂支架外固定组(A组)和DHS组(B组),每组患者40人,两组患者一般资料没有显著差异(P>0.05),具有可比性。结果 80例患者骨折均骨性愈合,A组优良率92.00%,B组优良率75.00%,具有显著性差异(P<0.05)。A组手术平均时间及术中出血量均明显少于B组,两组并发症发生率无统计学差异(P>0.05)。结论改良穿针单臂外固定架术较DSH治疗股骨粗隆间骨折患者疗效更好,改良穿针固定可靠,能早期下地扶拐行走,手术时间短,术中出血量少,不需再次手术取内固定,适合高风险的老年体弱者,值得临床进一步推广应用。 Objective To compare the efficacy of improved single arm needle external fixator technique vs the dynamic hip screw(DHS) for intertrochanteric fractures. Methods Eighty cases of intertrochanteric fracture were divided into two groups,receiving the improved single arm needle external fixation(group A) and the DHS fixation(group B),respectively. Results Eighty patients with fractures were healed.The excellent rate in group A was 92.5%,higher than in group B(75.00%)(P〈0.05).The average operating time and operative blood loss in group A was significantly reduced as compared with in group B,but no statistically significant difference was found in the incidence of complications between two groups(P〉0.05). Conclusion Modified external fixator technique is superior to needle arm DSH for patients with intertrochanteric fractures.Improved single arm needle external fixator technique had reliable fixation.
