脑脊液标本结核分枝杆菌的快速培养 被引量:5

The rapid culture of Mycobacterium tuberculosis from CSF samples

摘要 目的建立结核性脑炎脑脊液中结核分枝杆菌快速培养的方法。方法在7H9液体培养基加入一定浓度的复苏因子蛋白,接种脑脊液后进行培养,并用不含复苏因子蛋白质的7H9培养做对照。分别在第0、6、11、16和30d检测培养物的吸光度(A)值;取培养物10μl涂7H11平板,培养后进行CFU计数、抗酸染色。同时将标本进行罗氏培养和涂片抗酸染色。结果结核分枝杆菌复苏培养阳性率、涂片阳性率分别为55.00%和30.00%,差异有统计学意义(P<0.05);复苏培养阳性率与罗氏培养阳性率35.00%比较,差异无统计学意义(P>0.05)。复苏培养的结核杆菌平均在第10d进入对数生长期,其A值与对照组比较差异有统计学意义(P<0.01)。结论结核杆菌复苏因子能促进脑脊液中结核杆菌的生长。 Objective To establish a method for the rapid culture of Mycobacterium tuberculosis from cerebrospinal fluid(CSF) samples from patients with tuberculous encephalitis.Methods Specimens from patients with tuberculous encephalitis were cultured in 7H9 broth by adding different concentrations of recombinant resuscitation-promoting factor(Rpf) proteins.Specimens were cultured in 7H9 without Rpf to serve as a control.Culture growth was checked on day 0,6,11,16,and 30 by examining A600 value,7H11 plate colony count with 10 μl,and acid-fast staining.Specimens were cultured in Lowenstein-Jensen(LJ) medium and subjected to acid-fast staining to serve as a control.Results Cultures with Rpf proteins tested positive for M.tuberculosis at a rate of 55.00% while smears tested positive at a rate of 30.00%,and the difference between the two was statistically significant(P0.05).LJ medium tested positive at a rate of 35.00% and did not differ significantly(P0.05) from the rate for cultures with Rpf proteins.In cultures with Rpf proteins,M.tuberculosis took 10 days to enter the logarithmic growth phase and its A600 value differed significantly(P0.01) from that of the control group.Conclusion Rpf proteins can effectively accelerate the growth of tubercle bacilli growth in CSF.
