摘要
目的观察针刺配合艾灸督脉治疗类风湿关节炎的临床疗效,以及对细胞因子的影响。方法将90例患者随机分为西药组、针刺组和针灸组,西药组30例口服来氟米特治疗;针刺组30例采用针刺治疗;针灸组30例在针刺基础上,加用艾盒熏灸督脉治疗。观察记录治疗前及治疗60 d后症状分级量化评分,并检测实验室指标(ESR、RF、CPR、IL-1β、TNF-α)。结果西药组总有效率为78.6%,针灸组总有效率为96.5%,针刺组总有效率为82.1%,针灸组总有效率明显优于西药组和针刺组(均P<0.05)。针灸组治疗后症状积分显著低于治疗前(P<0.05);与针刺组和西药组治疗后比较,针灸组改善患者主要次要症状体征积分明显(P<0.05)。三组患者治疗后ESR、RF、CPR、IL-1β、TNF-α结果均低于治疗前(P<0.05)。结论针刺配合艾灸督脉治疗类风湿关节炎的临床疗效确切。
Objective To Investigate the clinical efficacy of acupuncture plus moxibustion on Du meridian in treating rheumatoid arthritis and its effect on cytokines. Methods Ninety patients were randomly allocated to Western drug, control and treatment groups, 30 cases each. The Western drug group took leflunomide; the control group was treated with acupuncture; the treatment group received acupuncture plus box-moxibustion on Du meridian. The major and subsidiary symptoms and signs were scored using a symptom rating scale in the patients before treatment and at 60 days after. Laboratory indices （ESR, RF, CPR, IL-I[~ and TNF-o0 were measured. Results The total efficacy rate was 78.6% in the Western drug group,： 96.5% in the treatment group and 82.1% in the control group. The therapeutic effect was better in the treatment group than in the control group （P〈0.05）. The symptom score was lower after treatment than before in the treatment group. The difference was statistically significant （P〈0.05）. After treatment, the major and subsidiary symptoms and signs score was obviously lower in the treatment group than in the control group and in the Western drug group. The difference was statistically significant in both （P〈0.05）. ESR, RF, CPR, IL-1[~ and TNF-~ were lower after treatment than before in all the three groups. All the differences were statistically significant （P〈0.05）. the post-treatent differences were statistically significant as the treatment group was compared with the control group and the Western drug group （P〈0.05）. Conclusions Acupuncture plus moxibustion on Du meridian has a definite clinical effect in treating rheumatoid arthritis. It is convenient and easy to perform.
