脑内血肿DWI及GRE-T2*WI信号演变规律的MRI分析 被引量:3

MRI analysis of rule of DWI and GRE-T2*WI signal changes in intracerebral hematoma

摘要 目的探讨不同时期脑内血肿的DWI、GRE-T2*WI序列信号演变规律。方法回顾性分析75例经临床及CT确诊的脑内血肿患者的的MR图像,全部病例均采集MRI常规T1WI、T2WI序列及DWI序列图像,其中39例采集GRE-T2*WI序列图像,结合血肿形成时间、主要成分及其在常规T1WI、T2WI的信号特征对脑内血肿进行分期,并分析不同时期脑内血肿在DWI、GRE-T2*WI序列的信号演变规律。结果超急性、亚急性晚期脑内血肿在DWI序列表现为"中高周低",其中5例超急性期、10例亚急性晚期病例行GRE-T2*WI序列检查,同样表现为"中高周低"。急性期、亚急性早期脑内血肿DWI序列表现为"中低周高",其中10例急性期、9例亚急性早期病例行GRE-T2*WI序列检查,均为低信号。慢性期脑内血肿DWI序列信号不稳定,其中5例慢性早期病例行GRE-T2*WI序列检查,表现为"中高周低"。结论不同时期脑内血肿在DWI、GRE-T2*WI序列的信号演变存在规律性,结合MRI常规序列检查可提高诊断效能。 Objective To study the rule of signal changes of intracerebral hematomas on diffusion weighted imaging(DWI) and gradient echo pulse sequence-T2*WI(GRE-T2*WI).Methods MRI data of 75 patients with intracerebral hematomas proved by clinical diagnosis and CT were retrospectively analyzed.T1WI,T2WI and DWI were performed on all of cases,and GRE-T2*WI was performed on 39 cases.Based on the time of hematoma formation,principal ingredient of hematoma and signal features on T1WI and T2WI,intracerebral hematomas were classified by stages in order to analyze the evolved rule of signal in intracerebral hematomas on DWI and GRE-T2*WI.Results In hyperacute and later subacute stage,hematomas were mainly hyperintensity in center and hypointensity on edge on DWI.GRE-T2*WI was performed on 5 cases of hyperacute stage and 10 cases of later subacute stage,and there were also hyperintensity in center and hypointensity on edge.The signals were hypointensity in center and hyperintensity on surrounding edge in acute stage and early subacute stage on DWI.GRE-T2*WI was performed on 10 cases of acute stage and 9 cases of early subacute stage,and there was mainly hypointensity.The signals of hematomas were variable in chronic stage on DWI.GRE-T2*WI was performed on 5 cases of early chronic stage,and there were hyperintensity in center and hypointensity on edge.Conclusions On DWI and GRE-T2*WI,the signal intensity of intracerebral hematoma has a rule in different period.Combining with the routine MRI,DWI and GRE-T2*WI may be helpful to the diagnosis of intracerebral hematoma.
