摘要
目的探讨静脉注入硫酸镁对蛛网膜下腔出血(SAH)后脑血管痉挛的防治作用。方法采用"枕大池二次注血法"制作兔SAH模型,SAH+硫酸镁组予硫酸镁股静脉输入,SAH组和没有SAH的对照组则予等量生理盐水。术前和术后全程观察实验动物的饮食及神经功能变化情况。在SAH前和SAH后第1、7天各组动物行椎基底动脉造影,利用HE染色及光学显微镜分别观察各组处理后第8天兔基底动脉血管形态学及显微结构改变。结果 SAH组兔摄食量逐渐下降,神经功能障碍逐渐加重;SAH+硫酸镁组摄食量稍有下降,神经功能障碍较轻,且逐渐好转。在SAH后第7天通过血管造影SAH+硫酸镁组椎基底动脉血管明显大于SAH组(P<0.01),与正常组差异无显著性(P>0.05)。组织学检查发现SAH组基底动脉内弹力膜明显迂曲皱折,血管平滑肌肥大扭曲变形,部分坏死,胞浆浓染,胞核固缩,外膜细胞增多。结论硫酸镁能明显逆转SAH后的脑血管痉挛,且有良好的血管保护作用。
Objective To investigate the effectiveness of intravenous magnesium sulfate on cerebral vasospasm in the experimental SAH rabbit model.Methods The "double-SAH model" was used.Normal saline(SAH group) or magnesium sulfate in normal saline(SAH+MG group) was infused into femoral vein for 7 days.Control rabbits without SAH also received normal saline(control group).The diet and nervous function of every animal before and after the operation were recorded.Angiography of the vertebrobasilar artery was performed on before and 1d and 7d after intravenous administration of magnesium sulfate.HE stain were used to observe the morphological changes of basilar arteries after 8d.Results SAH group rabbit showed reduced appetite and aggravated neural dysfunction,while these situations were mild in SAH+MG group.The Serum magnesium concentration was significantly increased in the SAH+MG group.The diameters of the vertebral artery on 7d were significantly decreased in the SAH group compared with SAH+MG group(P〈0.01).Histological examination revealed that the intima and internal elastic lamina show folding and corrugation and the edematous change with infiltration of inflammatory cells in the SAH group,but these changes were slight in the SAH+MG group.Conclusion Intravenous magnesium sulfate can reverse the cerebral vasospasm and has neuroprotective effect.
