摘要
目的探讨胫骨Pilon骨折手术治疗措施。方法 43例Pilon骨折患者根据Ruedi-Allgowor分型：I型（累及关节面无移位劈裂骨折）12例,Ⅱ型（型为累及关节面并有移位的劈裂骨折,但无粉碎）18例,Ⅲ型（累及干骺端及关节面的压缩、粉碎性骨折）13例。根据骨折类型分别采用不同治疗方法治疗,手术切开复位内固定23例,外固定架固定8例,外固定架结合有限内固定治疗14例。结果 43例Pilon骨折患者中,按Mazur踝关节评价分级系统,优21例,良18例,中3例,差1例;优良率90.7%。结论临床应根据骨折具体情况,软组织损伤程度及伤口开放情况选择不同的治疗方式。
Objective Pilon tibial fracture surgical treatment measures.Methods 43 cases Pilon fracture patients according to Allgowor points type： Ruedi-type I（involving the joint surface without shift fracturing fractures） in 12 cases,Ⅱ type（type for involving the articular surface and have displaced fracture fracturing,but without crushing） Ⅲ type（18 cases,involving the dry Hou end and articular surfaces comminuted fracture） compressed,13 cases.According to fracture type we adopt different treatment therapy,surgical incision and open reduction and internal fixation in 23 cases,external fixation frame fixed 8 cases,external fixation combined with limited internal fixation for plane 14 cases.Results 43 cases Pilon fracture patients,press Mazur ankle evaluation classification system,excellent 21 cases ＂good＂ in 18 cases,3 cases,poor 1;Rate was 90.7%.Conclusion clinical should according to fracture specific circumstances,soft tissue injury degree and the wound and opening-up choose different treatments.
