脊髓灰质炎病毒灭活疫苗在中国幼儿中加强免疫的血清学效果观察 被引量:1

Survey of Serological Effect on Boost Vaccination of Inactivated Poliovirus Vaccine for Infants

摘要 目的评价脊髓灰质炎(脊灰)病毒灭活疫苗(Inactivated Poliovirus Vaccine,IPV)用于中国幼儿加强免疫的血清学效果。方法对176例经过2、3、4月龄3剂IPV基础免疫的婴儿,18月龄时进行1剂IPV加强免疫。采集加强免疫前、后血样,采用微量中和试验检测血清中Ⅰ、Ⅱ、Ⅲ型脊灰中和抗体(Neutralizing Antibody,NA)滴度,进行血清学效果评价。结果加强免疫前,婴儿脊灰Ⅰ、Ⅱ、Ⅲ型NA阳性率分别为88.1%、82.9%、81.8%,抗体几何平均滴度(Geometric Mean Titer,GMT)分别为1∶37.6、1∶37.5、1∶36.2;加强免疫后1个月,婴儿脊灰Ⅰ、Ⅱ、Ⅲ型NA阳性率均为100%,GMT分别达到1∶2051.6、1∶1518.5、1∶4482.3。加强免疫后,婴儿脊灰Ⅰ、Ⅱ、Ⅲ型NA≥4倍增长率分别为92.6%、83.0%、88.6%。基础免疫前血清NA阴性者和阳性者在加强免疫后比较,脊灰3个型NA阳性率都达到了100%,两组间抗体GMT(tⅠ=0.539,PⅠ=0.588;tⅡ=0.592,PⅡ=0.552;tⅢ=-1.054,PⅢ=0.282)及≥4倍增长率(χ2Ⅰ=1.044,PⅠ=0.307;χ2Ⅱ=3.574,PⅡ=0.059;χ2Ⅲ=0.042,PⅢ=0.913)差异无统计学意义。结论 IPV基础免疫后,18月龄幼儿加强免疫一剂IPV,血清学效果良好。 Objective To evaluate immune effect following boost vaccination of inactivated poliovirus vaccine(IPV).Methods 176 children primarily immunized with one dose IPV were boosted with one dose IPV in 18 months old.The sera of pre-boost and post-boost immunization were collected and subject to micro-neutralization test to evaluate immune effect.Results The sera-positive rates of pre-boost vaccination are 88.1%,82.9%,81.8% for type 1,2,3 respectively.The geometric mean titer(GMT)of pre-boost vaccination are 1:37.6,1:37.5,1:36.2 for type 1,2,3 respectively.After boost vaccination,the sera-positive rates are 100% for all of three types.The GMTs reach to 1:2051.6,1:1518.5,1:4482.3 for type 1,2,3 respectively.The 4-fold increase rates are 92.6%,83.0%,88.6% for type 1,2,3,respectively.The immune responses were compared between pre-primary positive and negative children groups after boost vaccination.The sera-positive rates of post-boost vaccination in both groups reach to 100% for all of three types.No significant differences were found for the antibody levels(tⅠ=0.539,PⅠ=0.588;tⅡ=0.592,PⅡ=0.552;tⅢ=-1.054,PⅢ=0.282)and the 4-fold increase rate(χ2Ⅰ=1.044,PⅠ=0.307;χ2Ⅱ=3.574,PⅡ=0.059;χ2Ⅲ=0.012,PⅢ=0.913)between two groups.Conclusion?The effect of boost vaccination of one dose IPV in 18-month children is satisfied.
