摘要
目的探讨1.5T磁共振在颈动脉粥样硬化诊断中的价值。方法用1.5T磁共振成像仪对31例临床诊断为颈动脉粥样硬化的患者行颈动脉高分辨扫描,包括四翻转脉冲(QIR)T1W I、快速自选回波(FSE)T2W I/PDW I和三维时间飞跃法(3D-TOF)序列,分析颈动脉粥样硬化的MR I表现及信号特征。结果 MR I清晰显示了31例78处颈动脉粥样硬化斑块,其中,引起管腔轻度狭窄48处,中度狭窄26处,重度狭窄2处,闭塞2处。43处斑块存在明显的脂质坏死核心信号,8处含钙化信号,7处含出血信号,2处伴血栓形成、管腔闭塞。28处见厚纤维帽信号,35处见薄纤维帽信号,15处斑块表面不光整。结论 1.5T磁共振可用于颈动脉粥样硬化的检测和定性分析,为临床治疗方案的选择及随访提供依据。
Objective To study the capability in diagnosis of carotid atherosclerotic plaque with 1.5T MR scanner.Methods Bilateral carotid arteries in thirty-one patients with atherosclerosis were examined in a 1.5T MR scanner.The pulse sequences included QIR-T1WI,FSE-T2WI/PDWI and 3D-TOF.The appearance and signal characteristics of plaques were analyzed.Results 31 patients with 78 lesions of carotid arteriosclerotic plaques were demonstrated clearly by MRI,including 48 lesions with mild stenosis,26 lesions with moderate stenosis,2 lesions with severe stenosis and 2 lesions with complete obliteration.Definite lipid core,calcification,hemorrhage and thrombosis were found in 48,8,7,2 plaques respectively.28 plaques presented the character of thick fibrous cap,while 35 plaques showed character of thin fibrous cap,and the surface of 15 plaques were rough.Conclusion High resolution multiple sequences imaging in 1.5T MR scanner can be used for detection and qualitative analysis of the carotid atherosclerotic plaque.It will be useful for the clinical treatment design and follow-up.
