摘要
目的:探讨临床药师在风湿性心脏病治疗中进行药学服务的途径和方式方法,提高风湿性心脏病治疗效果,改善患者的依从性。方法:以1例风湿性心脏病患者的治疗过程为例,药师根据相关防治指南,分析患者具体病理生理特点,与临床医师共同商讨制定个体化治疗方案,并对患者实施全程的药学监护,包括疗效观察、不良反应监测和患者用药教育等。结果与结论:临床药师参与临床合理用药,旨在优化治疗方案,确保患者用药安全与有效。
Objective： To investigate the ways and methods of clinical pharmacists providing pharmacy services in the treatment of rheumatic heart disease to improve the services,treatment and compliance.Methods： In one case of rheumatic heart disease in the course of treatment,for example,based on the relevant prevention and treatment guidelines,pharmacists analyzed the specific pathophysiological characteristics of the patient,discussed the development of individualized programs with clinicians,and implemented the full range of pharmaceutical care for the patient,including efficacy observation,adverse drug reaction monitoring and patient education.Results and Conclusion： Clinical pharmacists＇ participation in rational drug use can optimize the treatment and ensure the safety and efficacy of drug in patients.
