长期血液透析导管相关并发症分析及对策 被引量:6

摘要 目的提高对长期血液透析导管相关并发症的认识及诊治水平,改善血液透析患者透析质量及预后。方法回顾性分析2009年6月~2010年6月采用Seld inger技术留置深静脉长期血液透析导管20例患者临床资料。结果置管后即刻透析导致皮下隧道出血1例,经填塞明胶海绵碎片压迫治愈。导管远端在血管腔内折返1例,在X线监视下重新调整导管位置至满意。透析导管相关性感染3例,其中1例经抗感染治愈,2例抗感染治疗无效拔管。导管通路功能不良3例,其中1例系导管尖端纤维蛋白外鞘形成予以拔管,其余2例需定期以尿激酶溶栓。结论带涤纶套的长期血液透析导管(tunneled cuffed catheter,TCC)可作为血液透析患者理想的血管通路,但应注意其适应证。感染和导管通路功能不良是其主要并发症,掌握正确的操作方法可有效降低风险,减少并发症。
