腕式睡眠监测仪在阻塞性睡眠呼吸暂停低通气综合征诊断中的应用 被引量:5

Clinical use of watch portable monitor in the diagnosis of obstructive sleep apnea hypopnea syndrome

摘要 目的评估腕式睡眠监测仪(Watch-PAT 200)在诊断阻塞性睡眠呼吸暂停低通气综合征(OSAHS)中的价值。方法于2010年7~12月对36例疑似OSAHS患者同步使用Watch-PAT 200和整夜多道睡眠图(PSG)两种方法进行监测。记录呼吸暂停低通气指数(AHI)、最低动脉血氧饱和度(lowest arterial oxygen saturation,LSaO2)以及平均动脉血氧饱和度(mean arterial oxygen saturation,MSaO2)。两种方法测得数据采用线性相关回归及Bland-Altman一致性分析。计算两种监测方法的符合率、敏感性和特异性。结果两种仪器测量AHI、MSaO2和LSaO2指标的相关性和一致性较好。两种监测方法的平均醒睡符合率为90.7%,平均基本睡眠结构符合率为86.3%,Watch-PAT200平均敏感性为87%,平均特异性为91%。结论 Watch-PAT200具有简便实用、结果可靠的特点。 OBJECTIVE To assess the accuracy and clinical efficacy of a wrist-worn portable device(Watch-PAT200)in diagnosis of obstructive sleep apnea hypopnea syndrome(OSAHS).METHODS Thirty-six participants with suspected OSAHS underwent full polysomnography(PSG)and Watch-PAT200 monitoring simultaneously.All PSG were manually scored according to the scoring manual of the Chinese Academy of Sleep Medicine(CASM 2002)and the Watch-PAT 200 data were analyzed by the automatic algorithm.The apnea hypopnea index(AHI),lowest arterial oxygen saturation(LSaO2)and mean arterial oxygen saturation(MSaO2)were recorded.Pearson's correlation coefficient and Bland-Altman analysis were used to assess the correlation and agreement between PSG data and Watch-PAT200 data.Then the diagnosis accordance rate,sensitivity and specificity between Watch-PAT200 and PSG were compared.RESULTS Average PSG-AHI and average PAT-AHI were(31.2±22.4)/h and(26.8±21.2)/h respectively.There was a high correlation of AHI(r=0.81,P0.001)between the two monitoring methods.MSaO2 and LSaO2 also had good correlation and agreement.Average microarousal accordance rate of two test methods was 90.7%,average sleep structure accordance rate was 86.3%.The sensitivity and specificity of Watch-PAT200 in diagnosis of OSAHS were 87% and 91% respectively.CONCLUSION Watch-PAT 200 may provide an accurate and clinical effective portable monitoring method for the diagnosis of sleep apnea.
