阻塞性睡眠呼吸暂停低通气综合征合并高血压患者血浆和呼出气冷凝液AngⅡ和ET-1检测的意义 被引量:8

摘要 目的探讨阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea-hypopnea syndrome,OS-AHS)合并高血压患者血浆和呼出气冷凝液(exhaled breath condensate,EBC)中血管紧张素Ⅱ(angiotensinⅡ,AngⅡ)和内皮素-1(endothelin-1,ET-1)的变化及意义。方法选择中、重度OSAHS合并高血压患者21例,OS-AHS血压正常组26例和健康对照组22例。3组均进行多导睡眠监测(polysomnograply,PSG),采用放射免疫法测定血浆和呼出气冷凝液中AngⅡ和ET-1水平。结果 (1)OSAHS合并高血压组呼出气冷凝液中AngⅡ值高于健康对照组(P<0.01),而OSAHS血压正常组呼出气冷凝液中AngⅡ值与健康对照组相比差异无统计学意义(P>0.05);呼出气冷凝液中的ET-1水平3组差异无统计学意义(P>0.05)。(2)OSAHS合并高血压组和OSAHS血压正常组血浆AngⅡ均高于对照组(P<0.05,P<0.01),且OSAHS合并高血压组高于OSAHS血压正常组(P<0.05);OSAHS合并高血压组和OSAHS血压正常组血浆ET-1水平均高于健康对照组(P<0.05),但OSAHS合并高血压组和OSAHS血压正常组之间差异无统计学意义。(3)3组患者呼出气冷凝液中AngⅡ和ET-1与血浆中AngⅡ和ET-1之间无明显相关性(r=0.480,P=0.397;r=-0.107,P=0.381)。(4)通过多元逐步回归分析显示,血浆ET-1水平与OSAHS的严重程度相关(B=6.810,β=0.372,P=0.002);呼出气冷凝液和血浆中AngⅡ均与舒张压相关(B值分别为0.110、0.863,β值分别为0.245、0.441,P值分别为0.042和0.048)。结论呼出气冷凝液和血浆中AngⅡ均与舒张压相关,而且OSAHS合并高血压患者呼出气冷凝液中AngⅡ高于健康者,但OSAHS血压正常患者呼出气冷凝液中AngⅡ值与健康者无明显差异,推测监测呼出气冷凝液中AngⅡ的变化可能为OSAHS合并高血压患者的诊断和病情判断提供帮助。
