摘要
目的通过分析妊娠合并胎儿单脐动脉的临床情况及分娩结局,探讨产前发现单脐动脉对妊娠及产科处理的影响。方法回顾性分析南宁市妇幼保健院2008年3月至2010年9月收治的21例产前超声诊断为胎儿单脐动脉患者的临床资料。结果 17例为单纯性单脐动脉,其中2例发生晚期流产,15例活产;4例合并结构畸形,死胎引产。全部病例均经胎盘及脐带病理证实。结论产前发现单脐动脉,需做系统超声检查,在排除胎儿合并其他畸形或染色体异常时,可以考虑继续妊娠,但尚须密切注意胎儿发育情况和产程。
Objective To investigate the influence of single umbilical artery discovered before delivery on pregnancy and obstetric treatment through analyzing clinical conditions and delivery outcome.Methods Twenty-one cases of ultrasound-diagnosed fetal single umbilical artery admitted to Nanning Maternal and Child Health hospital between March 2008 and September 2010 were retrospectively analyzed.Results Among the 17 cases of isolated single umbilical artery,late abortion occurred in 2 and live births in 15.Four cases were associated with fetal structural abnormalities and underwent fetal death induction.All cases were confirmed by pathological examination of the placenta and umbilical cord.Conclusion Systematic ultrasound examination should be conducted if prenatal single umbilical artery is discovered.When other associated malformations or chromosomal abnormalities are eliminated,continuing pregnancy can be considered,but the fetal growth condition and birth process should obtain close attention.
