摘要
目的探讨鼻咽癌颈部淋巴结转移特点以及和正常淋巴结两者ADC值之间的差异。方法应用Toshiba at-las 1.5 TMR扫描仪,58例鼻咽癌淋巴结转移患者和22例颈部淋巴结正常者进行DWI序列扫描,得出颈部淋巴结转移个数和区域,并计算两组淋巴结的ADC值。结果 58例患者中51例(91.1%)合并颈部淋巴结转移,咽后淋巴结转移49例(84.9%)。鼻咽癌患者转移淋巴结全淋巴结ADC值和最大层面ADC值(×10-3mm2/s)分别为0.783±0.086、0.678±0.134,对照组为0.955±0.126、0.934±0.157,两者差异显著(P<0.01)。ADC值取值0.863 mm2/s时,区别鼻咽癌转移淋巴结和对照组淋巴结的敏感性为85.3%,特异性为72.1%。结论鼻咽癌淋巴结转移有其分布规律,DWI序列有助于鉴别是否为转移淋巴结。
Objective To explore the characteristic of metastatic nodes in nasopharyngeal carcinoma(NPC) patients,and the differences of this nodes from benign ones with apparent diffusion coefficient(ADC) values.Methods DWI scan were performed in 58 patients with NPC and 22 cases with normal nodes by Toshiba 1.5T MR scanner,and sum all the nodes were shown and calculated the ADC value.Results Cervical metastatic nodes were shown in 51 cases(91.1%),while retropharyngeal metastatic lymph nodes in 49 cases(84.9%).ADC values of the whole node and a single largest section between NPC and benign was 0.783±0.086,0.678±0.134 and 0.955±0.126,0.934±0.157,respectively.By using the single section technique,setting the optimal ADC threshold value at 0.863mm2/s,the sensitivity for distinguishing NPC and benign was 85.3%,and the specificity was 72.1%.Conclusions Metastatic lymph nodes of NPC has its regularity,and using ADC value DWI technique might be a new helpful approach in the detection of this metastatic lymph nodes.
