摘要
目的评估多层螺旋CT(MSCT)对多导人工耳蜗植入术的运用价值。方法对20例重度或极重度感音性聋患者进行术前CT评估和11例已成功行多导人工耳蜗植入的患者做MSCT横断面靶扫描及内耳三维重建。结果语前聋组中显示8例内耳结构基本正常,6例中耳乳突化脓性炎症;语后聋组中显示:3例内耳结构基本正常,其中1例前庭导水管扩张;1例耳硬化症,2例中耳乳突化脓性炎症。11例行多导人工耳蜗植入术观察植入电极均位于耳蜗内,其形态、植入深度及走向均清楚显示,无并发症。结论多层螺旋CT检查对临床多导人工耳蜗植入术术前病例的选择及术后耳蜗内植入电极的观察及并发症的检出均具有重要参考意义。
Objective To investigate the application value of multi-slice CT (MSCT) on multi-lead artificial cochlea implantation. Methods Pre-operation CT evaluation were undertaken on 20 cases of severe or extreme sensory deafness, and post-operative CT evaluation were applied to 11 cases of successful implantation of multi-lead artificial cochlea, using axial target scanning and 3-dimenssional reconstruction techniques with MSCT. Results In the pre-lingual deafness group, 8 cases were revealed to be basically normal internal ear, 6 cases to be chronic inflammatory tympanomastoiditis. In the post-lingual deafness group, 3 cases were put out to be basically normal internal ear, of which 1 case suffered from vestibular aqueduct syndrome; 1 case to be otosclerosis; 2 cases to be chronic inflammatory tympanomastoiditis. The electrodes of the 11 post-operative cases of artificial cochlea implantation were all in the cochlea, whose shape, implantation death and orientation were clearly demonstrated, with no complications. Conclusion Multi-slice CT examination was of important reference significance, when it comes to pre-operative case selection and post-operative observation of implanted electrodes of artificial cochlea and their complications in multi-lead artificial cochlea implantation.
