摘要
目的:探讨左卡尼汀治疗血液透析(HD)中低血压的疗效。方法:52例HD中出现低血压的患者随机分为治疗组与对照组各26例。全部患者禁食、高低钠序贯透析,输注高渗葡萄糖及氯化钠注射液、低温HD等治疗措施。治疗组于HD结束后静注左卡尼汀1.0 g,每周2次,疗程12周。记录HD前、HD开始后2 h与结束后血压值。比较患者用药前后血压变化。结果:应用左卡尼汀患者血液透析中血压较治疗前明显升高(P<0.05),升高幅度明显高于对照组(P<0.05);总有效率达69.23%,亦明显高于对照组(P<0.05),未出现不良反应。结论:左卡尼汀治疗HD中低血压安全有效。
Objective： To investigate the efficacy of L-carnitine in the treatment of intradialysis hypotension （IDH） in maintenance hemodialysis （MHD） patients. Method：52 MHD patients with IDH were enrolled and randomly assigned into 2 groups （ n = 26 ） ： treatment group and controlled group. The patients in the treatment group were treated with iv L-canitine 1.0 g twice per week after hemodialysis session for 12 weeks, and the patients in the controlled group were given the treatment such as fast, cool dialysis, hypernatric ＆ hyponatrie sequential dialysis or given hypertonie saline or hyperosmotic glucose. Their blood pressure was measured and recorded at pre-, post-hemodialysis and 2 hours after the beginning of hemodialysis. Their blood pressures were compared with L-carnitine before and after the treatment. Result： Their blood pressures more significantly rose （ P 〈 0.05 ） than those before the treatment with L-carnitine. The rising amplitude was significantly higher than those in the controlled group （P 〈 0.05 ）. The total effective rate being 69.23% was significantly higher than that of the controlled group （ P 〈 0. 05 ）. The side effects were not observed. Conclusion： L-carnitine was safe and effective for the treatment of IDH.
