EnCor真空辅助旋切技术对不可触及的可疑乳腺癌微小病灶的诊治分析 被引量:9

摘要 目的探讨EnCor真空辅助旋切技术诊治乳腺不可触及可疑微小癌的可行性。方法选取37例乳腺高频超声或钼靶X线摄片发现的单发可疑乳腺癌微小病灶患者,在高频超声实时引导下使用EnCor真空辅助旋切技术将病灶完全切除,继续扇形扩大切除周围超过1 cm边缘的乳腺组织送检。结果病理确诊为原位癌8例,浸润性导管癌4例。行保乳手术9例,乳腺癌改良根治术3例,11例获阴性切缘,1例为阳性切缘。术后穿刺创道及微创切口均未发现癌细胞残留。12例恶性病灶者平均随访18(2~34)个月,25例良性病灶患者术后平均随访7(3~12)个月,彩超随访无肿瘤残留或复发。结论超声引导下的EnCor真空辅助旋切技术准确、可靠,可作为切检临床不可触及可疑乳腺癌微小病灶的首选方法之一。 Objective To explore the feasibility of diagnosis and treatment on nonpalpable small lesions of suspicious breast cancer by EnCor vacuum-assisted biopsy techniques.Methods 37 cases with single small lesions of suspected breast cancer were detected by high frequency ultrasound or mammography X-ray.Lesions were resected completely and more than 1 cm fan-shaped edge surrounding lesions were removed expandly using EnCor vacuum-assisted biopsy technique under the guide by real-time high frequency ultrasound.Results 8 cases were diagnosed as carcinoma in situ and 4 cases as infiltrating ductal carcinoma.11 cases got negative surgical margin and 1 case with positive surgical margin.No cancer cell was found in punctio-tract and incision after breast-conserving surgery or modified radical mastectomy.12 cases of malignant tumor were followed for 18(2 ~34)months,and the others of the benign tumors were followed for 7(3~12) months.No tumor residue or recurrence was detection during ultrasound follow-up.Conclusion Ultrasound-guided EnCor vacuum-assisted biopsy technique is accurate and reliable to be used as one preferred method of resection and biopsy on clinical nonpalpable small lesions of suspicious breast cancer.
