摘要
目的探讨高频超声在肘管综合征病变中的临床应用价值。方法对39例经临床和神经电生理拟诊为肘管综合征(CTS)的患者(40肘)为研究对象(患肢组),20名健康志愿者40只肘为正常对照组,进行肘部超声检查,并将患肢组检查结果与术前肌电图检查结果和术中所见进行比较。结果高频超声对尺神经卡压的检出率较高,对照组与患肢组尺神经厚度肿胀率和横截面积(CSA)肿胀率有显著变化,差异有统计学意义(P<0.05),患肢组尺神经的CSA与运动传导速度(MNCV)呈负相关(r=-0.933,P<0.01)。结论高频超声检查能实时动态观察尺神经卡压征象并明确病因,对肘管综合征诊断具有重要参考价值,为临床提供了一种简单、可靠的无创检查方法。
Objective To explore the value of high-resolution ultrasonography(US) in diagnosis of cubital tunnel syndrome.Methods 39 patients with cubital tunnel syndrome(CTS,40 elbows) were selected as limb group and 15 healthy volunteers were selected as normal control group.All subjects underwent Ultrasound(US) of the ulnar nerve.The findings of US measurement were compared with those of intra-operative and pre-operative electromyography.Results Detection of ulnar nerve compression with High-frequency ultrasound was high in both of limb group and control group.The thickness swelling rate of the ulnar nerve and the cross-sectional area(CSA) showed a significant difference between two groups(P0.05).The CSA and the ulnar nerve motor conduction velocity(MNCV) in group Limb showed significantly negative correlation(r=-0.933,P0.01).Conclusion High-frequency ultrasound is a painless,non-invasive,convenient way in dynamic real-time observation of signs of ulnar nerve compression.It has important reference value for diagnosis of cubital tunnel syndrome.
