摘要
目的分析综合ICU神经外科危重患者发生下呼吸道感染的相关危险因素并寻求相应的护理措施。方法回顾性分析医院ICU 2007-2009年收治的547例住院时间>7 d患者病例资料、化验检查报告、影像学检查报告、生物学检查报告等,并以神经外科危重患者246例作为观察组,以同期收治的其他科室危重患者321例为对照组进行对比研究。结果神经外科危重患者发生医院内下呼吸道感染的89例,占36.0%;痰培养结果共分离菌株127株,革兰阴性杆菌91株,占71.7%,革兰阳性球菌14株,占11.0%,真菌15株,占11.8%,其他7株,占5.5%;对照组患者发生下呼吸道感染的31例,占9.7%,痰培养结果共分离菌株47株,革兰阴性杆菌31株,占65.9%,革兰阳性球菌6株,占12.8%,真菌7株,占14.9%,其他3株,占6.4%。结论综合ICU神经外科危重患者下呼吸道感染的发生率明显高于对照组患者(P<0.01),两组患者痰细菌培养结果及药敏试验结果差异无统计学意义。
OBJECTIVE To analyze the high risks of neurosurgical critical patients with lower respiratory tract infection in the general ICU, and find out the appropriate measures of prevention and control. METHODS A retrospective analysis was carried out in ICU from Jun 2007 to Dec. 2009, admitted 547 patients in study, with clinical data, laboratory inspection reports, imaging inspection report, biological inspection report （bacterial culture ＋ sensitivity test） etc. 246 cases of critical neurosurgical patients in the observation group,and 321 cases of other departments in the same time as the control group. RESULTS The incidence of neurosurgical critical patients with lower respiratory tract infection 89 eases （36. 0%）, sputum culture results were 127 bacterial strains：Gram-negative bacillus 91 （71. 7%）, 14 （11. 0%）strains of gram-positive cocci, fungi of 15 （11. 0%） strains,and the other 7（5.5%）. The control group of patients with lower respiratory tract infection 31（9.7%） cases, sputum culture results were 47 bacterial strains ： 31 （65.9%） strains of Gram-negative bacillus, 6 （12.80%） strains of Gram-positive cocci, fungi of 7（14. 9%）, and the other 3 （6.4%）. CONCLUSION The incidence of lower respiratory tract infection in neurosurgical critical patients is significantly higher （P〈0.01）. The results of sputum bacterial culture and antibiotic susceptibility test in two groups have no significantly differentce （P〉0.05）.
