摘要
目的观察坎地沙坦酯联合比索洛尔及胺碘酮治疗高血压病合并持续性心房颤动的临床疗效。方法 96例高血压病合并持续性心房颤动患者随机分为常规治疗组和坎地沙坦组,每组48例,常规治疗组给予胺碘酮及比索洛尔治疗,坎地沙坦组在此基础上加用坎地沙坦酯,随访6、12个月后比较两组患者窦性心律维持率、左心房内径缩小等指标。结果随访6个月,坎地沙坦组窦性心律维持率为91.3,常规治疗组窦性心律维持率为83.3;随访12个月,坎地沙坦组窦性心律维持率为85.4,常规治疗组窦性心律维持率为75.0,两组同期窦性心律维持率差异有统计学意义(P均<0.05)。两组患者12个月后左心房内径均缩小,各组治疗前后左房内径差异有统计学意义(P<0.05),坎地沙坦组左房内径缩小更明显,治疗后两组左房内径差异有统计学意义(P<0.05)。结论坎地沙坦酯联合比索洛尔及胺碘酮能有效提高高血压病合并持续性心房颤动患者窦性心律维持率及缩小左房内径。
Objective To observe the curative effect of candesartan cilexetil combining with bisoprolol and amiodarone on hypertension accompanied by persistent atrial fibrillation.Methods The patients with hypertension accompanied by persistent atrial fibrillation（n=96） were randomly divided into the routine treatment group and candesartan group（each n=48）.The routine group was treated with amiodarone and bisoprolol,and the candesartan group was treated with candesartan cilexetil besides amiodarone and bisoprolol.The maintenance rate of sinus rhythm and narrowing situation of left atrial diameters were compared between two groups after follow-up for one year.Results After 6 months of follow-up,the maintenance rate of sinus rhythm was 91.3% in the candesartan group and 83.3% in the routine group,and after 12 months of follow-up,it was 85.4% in the candesartan group and 75.0 % in the routine group.The difference between two groups in the same period was statistically significant（all P0.05）.After 12 months,the left atrial diameter was reduced in two groups and more obvious in the candesartan group.The difference was statistically significant（P0.05） before and after the treatment in the same group,and was statistically significant（P0.05） between two groups after the treatment.Conclusion Candesartan cilexetil combining with bisoprolol and amiodarone can effectively increase the maintenance rate of sinus rhythm and reduce the left atrial diameter in the patients with hypertension accompanied by persistent atrial fibrillation.
