代谢综合征大鼠左心室应变及应变率与心外膜脂肪的相关性 被引量:3

Correlations of left ventricular strain and strain rate with epicardial adipose in rats with metabolic syndrome

摘要 目的探讨应变及应变率分析对代谢综合征(MS)大鼠心脏收缩功能的评估价值及其与心外膜脂肪的关系。方法 6周龄雄性SD大鼠16只,随机分为MS模型组8只、对照组8只。给予模型组大鼠小剂量链脲佐菌素、N硝基L精氨酸甲酯盐酸盐联合高脂、高盐、高糖饲养6周。实验开始及结束时测定大鼠的体质量、空腹血糖、胰岛素、血脂指标、口服糖耐量及血压水平,心脏超声检测大鼠左心室收缩期应变及应变率。实验结束时分离心外膜脂肪。结果 饲养6周后,模型组大鼠体质量[(462±32)比(406±49)g]、空腹胰岛素[(5.47±2.26)比(2.69±0.61)μg/L]、口服糖耐量曲线下面积[(1544.6±502.2)比(911.0±69.8)mg/(L·min)]、血浆三酰甘油[(0.66±0.44)比(0.28±0.07)mmol/L]、总胆固醇[(1.71±0.43)比(1.01±0.29)mmol/L]及低密度脂蛋白水平[(0.66±0.25)比(0.22±0.08)mmol/L]、收缩压[(163±26)比(118±12)mmHg]及舒张压[(122±16)比(91±17)mmHg]均高于对照组(P<0.05),而空腹血糖及高密度脂蛋白胆固醇水平与对照组比较无明显差异(P>0.05)。左心室外膜收缩期周向应变及应变率均低于对照组[(-7.20±1.46)%比(-9.87±1.93)%],(-1.56±0.35)比(-2.19±0.49)s-1;均P<0.05],心外膜脂肪质量高于对照组[(0.49±0.13)比(0.35±0.08)g,P<0.05]。心外膜脂肪质量与心外膜周向应变及应变率均正相关(r=0.679,P<0.01;r=0.712,P<0.01)。结论 MS大鼠左心室早期局部收缩功能出现障碍并与心外膜脂肪密切相关。 Objective To investigate the availability of strain and strain rate analysis in the evaluation of alterations in systolic function in rats with metabolic syndrome (MS) and the relationship between heart function and epicardial adipose tissue (EAT). Methods Sixteen male SD rats of 6 weeks old were randomly divided into MS group and control group (n= 8, respectively). The rats in MS group were intervened with low-dose streptozocin and N-Nitro- L-arginine methyl ester, and fed with high-fat, high-sah and high sugar diet for six weeks. Body mass, fasting plasma glucose (FPG), fasting insulin (FINS) , plasma lipids levels, blood glucose after oral glucose tolerance test (OGTT), and blood pressure were examined respectively. Left ventricular systolic circumferential strain and strain rate were evaluated with eehocardiography. EAT was isolated at the end of the study. Results Comparetl with those of the controls, the body mass [(462±32) vs (406±49)g], areas under the glucose curves [(1544.6±502.2) vs (911.0±69.8) mg/(L· min)], plasma triglyceride [(0.66±0.44) vs (0.28±0.07)mmol/L], total cholesterol [(1.71±0.43) vs (1.01±0.29)mmol/L] and low-density lipoprotein [(0.66± 0.25) vs (0. 224-±0.08)mmol/L], systolic and diastolic blood pressure [{163±26) vs (118±12)mm Hg, (122±12) vs (91±17) mm Hg, respectively], and EAT mass [(0. 49±0.13) vs (0.35±0.13)g] of the rats in model group were all significantly higher than that of the controls (P〈0.05), while no differences in FPG [(5.15±2.35) vs (5. 76±0.73)mmol/L] and highdensity lipoprotein levels [(0. 72±0.20) vs (0.83±0. 18)mmol/L] were found between the two groups(P〉0.05)after being fed for six weeks. Left ventricular systolic epicardial circumferential strain and strain rate of MS rats were significantly lower than those of the controls [-(-7.20±1.46) vs (-9.87±1.93)% (-1.56±0.35) vs ( - 2.19±0.49) s -1, both P〈0.05]. EAT mass was positively correlated with left ventricular systolic epicardial cir cumferential strain and strain rate positively( r= 0. 679, P〈0.01 ; r= 0. 712, P〈0.01 ). Conclusion Early-im paired regional systolic function occurs in MS model rats, which is closely associated with EAT.
