应用拦石网输尿管镜下钬激光碎石术治疗输尿管上段结石 被引量:4

Treatment of upper ureteral calculi with intercept stone network combined with holmium:YAG laser lithotripsy under ureteroscopy

摘要 目的探讨应用拦石网输尿管镜下钬激光碎石术治疗输尿管上段结石的临床疗效。方法回顾性分析输尿管镜下拦石网配合钬激光碎石术治疗58例输尿管上段结石患者的临床资料。男36例,女22例,平均47.2岁。结石横径为3~23mm,纵径为3~25mm。结果本组单次碎石成功率93.1%(54/58)。2例结石上移入肾内,1例因输尿管上段迂曲而改行输尿管切开取石术,1例因斑马导丝无法进入输尿管上段而改为经皮肾镜钬激光碎石术。手术时间为25~90min,平均时间37.4min。术后住院时间为3~11天,平均4.7天。术后发生尿路感染2例,无输尿管穿孔等严重的并发症发生。结论应用拦石网输尿管镜下钬激光碎石术治疗输尿管上段结石安全、有效,拦石网能有效的阻止结石的上移、有助于结石的安全取出,值得临床推广应用。 Objectives To evaluate the effectiveness of the treatment of upper ureteral calculi with intercept stone network combined with holmium:YAG lithotripsy under ureteroscopy. Methods We retrospectively reviewed the data of 58 patients( 36 males and 22 females, with an average age of 47.2 years)with upper ureteral calculi, who were treated with intercept stone network combined with holmium:YAG laser lithotripsy under ureteroscopy. The width of calculus ranges from 3 to 23 mm, and the length 3 to 25 ram. Results The success rate of fragmentation in a single procedure was 93.1% (54/58). The calculi retrograded to renal in 2 cases. 1 case converted to open surgery because the upper ureter twisted. Operations in 1 case had to be changed to PCNL, because the zebra urological guidewire can not reach the upper ureter. The operation time was 25 - 90 mid, and the mean operative time was 37.4 mid. The postoperative hospital stay was 3 - 11 d, and the mean postoperative hospital stay was 4.7 d. Only 2 cases had urinary infection. No severe complications such as ureteral perforation were seen in this operation. Conclusions The treatment of upper ureteral calculi with intercept stone network combined with holmium laser lithotripsy under ureteroscopy is safe and effective, and the intercept stone network is an effective tool that blocks the upward movement of the stone and aids in safe stone removal. This therapeutic method and tool are worth recommending in clinical practice.
