摘要
目的探讨高频彩色多普勒超声对上肢血管瘤诊断和治疗的指导价值。方法对2006年7月至2009年3月收治的12例上肢血管瘤应用日本产Aloka SSD prosoundα10型高频彩色多普勒超声进行检查,并对9例患者予以手术治疗。手术后3周进行复诊观察手术效果。结果对9例患者进行了手术治疗,高频彩色多普勒超声显示与手术所见完全相符。二维超声显示:肿瘤平均大小6.6cm×4.0cm,形态不规则,与周围组织分界不清,内为不规则低无回声暗区,似"蜂窝"状,可见扩张的血管或血窦内血栓形成及钙化,呈现强回声,后方有声影,压迫肿物或者变换体位时肿物可以增大。彩色多普勒见有丰富的血流信号,血流Alder分级均为Ⅲ级,异常血流区域平均大小7.2cm×5.0cm,比较二维超声范围增大。压迫肿物可见暗区扩张,血流信号增多。结论高频彩色多普勒超声对上肢血管瘤的定性及准确的定位诊断、侵袭范围以及对术中指导和手术后疗效的评价具有重要的临床价值。
Objective To investigate the instructive significance of high-frequency colour Doppler ultrasound (HCDU) in the diagnosis and treatment of angioma in the upper extremities. Methods From July 2006 to March 2009, 12 cases of angioma in the upper extremities underwent SSD prosound ctl 0 HCDU (Aloka, Japan) examinationm, and 9 of them underwent surgery. We observed the surgery outcomes 3 weeks after surgery. Results Images shown on HCDU were consistant with the visual we got in surgery. The two-dimensional ultrasonography demonstrated: the tumors were 6.6cm×4.0cm of irregular shape; no distinct border from surrounding tissues; the morphous of the tumors was disordered with the resemblence of honeycombs; dilated blood vessels or sinusoidal thrombus formation and calssification could be seen and caused strong echo followed by sound shadow; the tumor enlarged when it was pressed or the body position changed. Through HCDU, we could see strong signal of blood flow. According to Alder blood-flow classification, all the signals were grade III. The abnormal blood-flow area was 7.2cm^5.0cm on average which was bigger than that of the two-dimensional ultrasonography. When the tumor was compressed, we could see the enlarged dim area and a stronger blood- flow signal. Conclusions High-frequence color Doppler ultrasonography examination has important instructive significance to the qualitative confirmation and location, detecing the range of lesio, operating and post-operative evaluation.
